Dcd Liver Preservation

DCD liver preservation comprises the methods used to maintain liver viability after donation following circulatory death, when a period of warm ischemia can compromise graft quality. Preservation typically combines controlled cold storage with machine perfusion, which circulates an oxygenated preservation solution through the liver to supply substrates, remove waste, and assess function before transplantation. These bioengineering approaches can reduce ischemia-reperfusion injury, support the use of marginal grafts, and provide measurable indicators of organ viability. Improved preservation may expand the donor pool while enabling more individualized decisions about graft suitability and transplant safety.

Technological advances have led to optimizing the conditions of preservation, along with a more accurate assessment of the donor heart before transplantation. We herein describe two innovative technologies currently applied for cardiac preservation: uniform cooling preservation and normothermic ex-situ heart perfusion.

The porcine model of liver normothermic machine perfusion (NMP), described here, can be successfully used to study NMP as a preservation strategy, a tool for viability assessment, and a platform for organ repair. It holds a high translational value, however it is technically challenging and labor-intensive.

Here we present a protocol describing oxygenated ex situ machine perfusion of donor liver grafts. This article contains a step by step protocol to procure and prepare the liver graft for machine perfusion, prepare the perfusion fluid, prime the perfusion machine and perform oxygenated normothermic machine perfusion of the liver graft.
