Central Activation Ratio

Central Activation Ratio (CAR) is a physiological measure that quantifies how fully the nervous system activates a muscle during a voluntary contraction. It is typically calculated by comparing torque produced voluntarily with the total torque produced when an electrically stimulated twitch is superimposed on that contraction: CAR equals voluntary torque divided by voluntary plus twitch torque, multiplied by 100. A lower ratio indicates central activation failure, in which neural drive does not recruit the muscle’s available motor units effectively. In medicine and rehabilitation research, CAR helps assess neuromuscular impairment, monitor recovery after injury or surgery, and evaluate interventions aimed at restoring muscle function.
