Massive Irreparable Tears

Massive irreparable tears are extensive tendon injuries, most commonly involving the rotator cuff, that cannot be reliably restored to their original attachment through conventional surgical repair. Severe tendon retraction, muscle atrophy, fatty degeneration, poor tissue quality, or chronic changes can prevent the torn tissue from reaching the bone and healing effectively. In medicine, recognizing these features guides treatment planning, including rehabilitation, pain management, tendon transfers, superior capsular reconstruction, or reverse shoulder arthroplasty when appropriate. Studying these tears helps clinicians match interventions to tissue condition, preserve shoulder function, and improve outcomes for patients with complex musculoskeletal injuries.
