In wässrigem Natriumhydroxid gehen zwei Aldehydmoleküle eine Additionsreaktion ein, um einen β-Hydroxyaldehyd zu bilden. Hierbei handelt es sich um eine basenkatalysierte Aldol-Additionsreaktion mit einem mehrstufigen Mechanismus.
Die Reaktion beginnt mit einem Enolisierungsschritt, bei dem das Hydroxidion den Aldehyd an der α Kohlenstoff reversibel deprotoniert, um ein resonanzstabilisiertes Enolat-Ion zu erzeugen.
Als nächstes ergibt die nukleophile Addition des α Kohlenstoffs des Enolats an die Carbonylgruppe des nicht umgesetzten Aldehyds ein Alkoxid-Ionen-Zwischenprodukt.
Durch die anschließende Protonierung des Alkoxidions wird ein β-Hydroxyaldehyd als Aldol-Additionsprodukt erzeugt. Hier ist das Gleichgewicht in Vorwärtsrichtung günstiger.
Bei α-substituierten Aldehyden verschiebt sich das Gleichgewicht jedoch aufgrund der sterischen Behinderung am Reaktionsort zurück zu den Vorläufern. 
Bei Ketonen begünstigt das Gleichgewicht den Ketonreaktanten und nicht das entsprechende Aldol-Additionsprodukt.

Die Reaktion findet bevorzugt mit einfachen Aldehyden statt, bei denen der α-Kohlenstoff einfach substituiert ist. Das Gleichgewicht der Reaktion mit disubstituierten Aldehyden und Ketonen verschiebt sich aufgrund der sterischen Wechselwirkungen am α-Kohlenstoff nach hinten zu den Reaktanten. Die trisubstituierten Aldehyde ohne α-Wasserstoffatom sind für die Aldoladditionsreaktion ungeeignet.

Der Mechanismus erfolgt in drei verschiedenen Schritten: Enolisierung, nukleophile Addition und Protonierung. Zunächst deprotoniert die Base, wie in Abbildung 2 dargestellt, den α-Kohlenstoff des Aldehyds, um ein Enolat-Ion zu erzeugen.

Abbildung 3 zeigt den anschließenden nukleophilen Additionsschritt. Hier fungiert das Enolat-Ion als Nukleophil und greift die Carbonylgruppe des nicht umgesetzten Aldehyds an, um ein Alkoxid-Ion-Zwischenprodukt zu bilden.

Schließlich wird, wie in Abbildung 4 dargestellt, das Alkoxidion protoniert, um einen β-Hydroxyaldehyd als Aldolprodukt zu bilden. Die im Produkt vorhandenen funktionellen Aldehyd- und Alkoholgruppen geben der Reaktion den Namen „Aldol“.

Base-catalyzed aldol addition forms a beta-hydroxy carbonyl compound from two carbonyl compounds in aqueous sodium hydroxide. The reaction is most favorable with simple aldehydes, especially when the alpha carbon is monosubstituted. When the aldehydes or ketones are disubstituted, steric crowding at the alpha carbon can push the equilibrium back toward the reactants. Trisubstituted aldehydes, which do not have an alpha hydrogen atom, are not suitable for this reaction.

The mechanism happens in three steps: enolization, nucleophilic addition, and protonation. First, the base removes a hydrogen from the alpha carbon of the aldehyde. This creates an enolate ion. An enolate is a resonance-stabilized ion that can act as a nucleophile.

Next, the enolate ion attacks the carbonyl group of a second, unreacted aldehyde. This nucleophilic addition forms an alkoxide ion intermediate. In the final step, the alkoxide ion is protonated. The product is a beta-hydroxy aldehyde, and the reaction gets the name aldol because the product contains both an aldehyde group and an alcohol group.
