Clinical Care Pathway

A clinical care pathway is a structured, evidence-informed plan that coordinates the assessment, treatment, and follow-up of patients with a specific condition or care need. It maps key decisions, interventions, responsibilities, and expected milestones across the patient journey, while allowing clinicians to adapt care when individual circumstances or outcomes differ. In clinical practice, care pathways support communication among multidisciplinary teams, reduce unnecessary variation, and promote timely, consistent care. They also provide a framework for evaluating outcomes, identifying gaps in service delivery, and improving the quality, safety, and efficiency of healthcare.

Structured protocols are necessary to provide answers on research questions in critically ill patients. The Simple Intensive Care Studies (SICS) provides an infrastructure for repeated measurements in critically ill patients including clinical examination, biochemical analysis and ultrasonography. SICS projects have specific focus but the structure is flexible to other investigations.

Development and Implementation of a Multi-Disciplinary Technology Enhanced Care Pathway for Youth and Adults with Concussion

A multidisciplinary carepath to standardize disparate concussion care was developed and implemented at the Cleveland Clinic. The Cleveland Clinic Concussion (C3) mobile application was used to enable the carepath through biomechanical outcomes characterizing cognitive and motor function. Patient outcomes improved while cost of care was reduced following implementation.
