Community Health Services

Community health services are organized healthcare programs that prevent disease, promote wellness, and provide treatment or support within the communities where people live. They work by coordinating clinicians, public health professionals, outreach workers, and local organizations to deliver health education, screening, vaccination, chronic disease management, and referrals, often through clinics, schools, homes, and mobile units. In clinical practice, these services improve access for underserved populations and connect individual care with population health goals. They also support early diagnosis, continuity of care, and culturally responsive interventions, helping reduce health disparities and strengthen community resilience.
