Patient Identification

Patient identification is the process of accurately matching an individual with their health records, orders, treatments, and diagnostic results, making it a foundation of safe clinical care. Healthcare professionals verify at least two patient identifiers, such as name and date of birth, and compare them with wristbands, electronic records, laboratory requests, or medication orders before providing care. This process helps prevent wrong-patient errors, including incorrect medications, procedures, tests, or documentation. Consistent identification practices support communication across clinical teams, strengthen electronic health record accuracy, and improve patient safety in hospitals, clinics, laboratories, and other healthcare settings.

Measuring Frailty in HIV-infected Individuals. Identification of Frail Patients is the First Step to Amelioration and Reversal of Frailty

Frailty syndrome is commonly seen in the aged and reflects multi-system physiological change. However, with reduced functional reserve and resilience frailty is also known to be common in the HIV infected population. This study outlined an easily administered screening test to identify HIV patients with frailty. When significant components of frailty are identified, clinicians will be able to focus on amelioration of the problem and promote reversion to the pre-frail state.
