Subchondral Bone Evaluation

Subchondral bone evaluation is the assessment of the bone immediately beneath articular cartilage to characterize its structure, integrity, and role in joint health. It combines imaging and, when appropriate, histologic or biomechanical analysis to examine features such as bone remodeling, thickness, density, sclerosis, cysts, and lesions. In medicine, this evaluation helps identify changes associated with osteoarthritis, trauma, and other joint disorders, while clarifying how alterations at the cartilage–bone interface influence pain and disease progression. These findings support diagnosis, monitoring, treatment planning, and research into therapies that preserve joint function.

This methodology article presents a software-assisted quantitative measurement protocol to quantify histologic subchondral bone thickness in murine osteoarthritic knee joints and normal knee joints as controls. This protocol is highly sensitive to subtle thickening and is suitable for detecting early osteoarthritic subchondral bone changes.
