Hypertrophy is an increase in individual cell size, while hyperplasia involves an increase in cell number. Both enlarge tissues or organs, but through different mechanisms. Hypertrophy increases cell volume and enhances work capacity through greater protein and organelle production within existing cells, whereas hyperplasia adds more cells to the tissue.

Mechanical stress on muscle fibers from resistance training activates the IGF-1/Akt/mTOR signaling pathway. This activation stimulates protein synthesis and increases production of contractile proteins, promoting muscle fiber growth. As a result, muscles enlarge in size and strength while remaining a reversible adaptation if training stops.

Physiological hypertrophy is a normal, reversible adaptation to increased workload or hormonal signals that enhances tissue function without causing harm. It occurs in response to consistent mechanical overload, such as resistance training in skeletal muscle. This type of hypertrophy improves the cell's work capacity and is reversible if the workload decreases.

Chronic hypertension forces the heart to pump against increased resistance, causing persistent strain on myocardial cells. This sustained effort enlarges cardiac muscle cells in the myocardium, initially preserving cardiac function. However, prolonged hypertrophy thickens and stiffens heart walls, impairing relaxation and filling, ultimately increasing the risk of heart failure.

Pathological hypertrophy results from chronic or abnormal stress and impairs organ function over time, whereas physiological hypertrophy enhances function and remains reversible. Pathological hypertrophy occurs in response to pressure or volume overload and diseases like chronic hypertension. Though initially compensatory, it eventually leads to tissue dysfunction and disease progression.

Anabolic hormones like testosterone stimulate physiological skeletal muscle growth and hypertrophy. In contrast, growth factors such as transforming growth factor-beta are often involved in pathological hypertrophy. These signaling molecules regulate protein synthesis and cell enlargement, determining whether hypertrophy is beneficial or harmful to tissue function.

Hypertrophy is one of several cellular adaptations to stress or demand, distinct from cellular adaptation introduction and atrophy, which involve cell size decrease. Understanding hypertrophy alongside other adaptations provides context for how cells respond to physiological and pathological challenges, forming a comprehensive framework for cellular response mechanisms.
