L’axe hypothalamo–hypophyse–surrénal régule normalement la production de cortisol.
Lorsque les niveaux de cortisol sont bas, l’hypothalamus libère l’hormone libératrice de corticotropine, ou CRH, qui stimule l’hypophyse antérieure à sécréter l’hormone adrénocorticotrope, ou ACTH.
L’ACTH signale alors à la zone fasciculée du cortex surrénalien de libérer du cortisol. L’augmentation des taux de cortisol dans le sang supprime la CRH et l’ACTH par rétroaction négative.
Dans le syndrome de Cushing, cet équilibre est perturbé. Une cause fréquente est l’utilisation prolongée de corticostéroïdes, qui réduit la CRH et l’ACTH même lorsque les taux de cortisol sont élevés.
La maladie de Cushing se développe lorsqu'un adénome hypophysaire produit un ACTH excessif et résiste à l'inhibition par rétroaction. Des niveaux élevés d’ACTH stimulent chroniquement les glandes surrénales à produire plus de cortisol.
