Chronic Spasm Management

Chronic spasm management is the long-term use of therapeutic strategies to reduce recurrent involuntary muscle contractions, pain, and functional limitation. In pharmacology, treatment may involve drugs that decrease excitatory motor signaling, enhance inhibitory neurotransmission, or reduce acetylcholine release at the neuromuscular junction, depending on the spasm’s cause and location. Clinicians select and adjust therapy according to symptom severity, treatment response, comorbidities, and adverse-effect risk. Effective management can improve mobility, sleep, and quality of life while limiting sedation, weakness, and other medication-related harms.

Myocardial lactate production (coronary arterial-venous difference in serum lactate level) during coronary spasm provocation testing is considered as a highly sensitive marker that reflects acetylcholine-induced myocardial ischemia due to microvascular spasm.
