תקציר

מבוא

פסוריאזיס היא מחלת עור דלקתית כרונית שאינה מדבקת המופעלת על ידי תגובה חיסונית לא מווסתת, הפוגעת בקרטינוציטים היוצרים בעיקר את האפידרמיס1, המאופיינת בהכפלה מהירה באופן חריג של קרטינוציטים עם היפרקרטוזיס ופרקרטוזיס. פסוריאזיס משפיעה על כ-3% מאוכלוסיית הבר בעולם2. נטל המחלה גדל עוד יותר על ידי מספר מחלות נלוות, כולל מחלות לב וכלי דם ותסמונת מטבולית הנגרמת על ידי התסמונת3.

האפידרמיס מורכב מחמש שכבות של קרטינוציטים ועובר שינוי מורפולוגי בתהליך התמיינות: השכבה הבסיסית, השכבה הספינוזום, השכבה הגרנולוסומית, השכבה הלוסידומית (המצויה על כפות הידיים והסוליות) ושכבת הקרנית המתוארת כאן מהמשטח הפנימי לחיצוני4. שינוי בהתמיינות האפידרמיס מוביל להפרעה במחסום העור, החשוב לפתוגנזה של מחלות דלקתיות בעור 5,6,7,8. קרטינוציטים ממלאים תפקיד חיוני בשמירה על מחסום אפידרמיס שלם למניעת אובדן מים ונגד טריגרים סביבתיים כגון חשיפה ל-UVB, אלרגנים ופתוגנים9. אנשים בריאים מראים איזון בין התפשטות תאי בסיס לבין דשדוש שכבת הקרנית, בעוד שמחלות עור מרובות כולל פסוריאזיס, מאופיינות בחוסר איזון של מנגנון מורכב זה10.

מעבר לתפקוד המחסום, קרטינוציטים הם גם מרכיב חיוני במערכת החיסון של העור. באימונופתוגנזה של פסוריאזיס, הפעלה של תאי T מסייעים מסוג 1 (Th1) ותאי T מסייעים מסוג 17 (Th17), מובילה לייצור מוגבר של IFN-γ ו-IL-17A, בהתאמה. ציטוקינים אלה גורמים לסינתזה מוגברת של כימוקינים (CCL20, CXCL1/2/8/9/10/11), פפטידים אנטי-מיקרוביאליים (BD2, LL37, S100A7/8/9/12) וגורמים דלקתיים אחרים (TNF-α, IL-6, IFN-β) בקרטינוציטים, מה שמוביל לגיוס של יותר Th1, Th17 ונויטרופילים לעור, מה שמגביר עוד יותר את ציר IL-17/IL23 11. הדיבור ההדדי בין קרטינוציטים לתאי חיסון אחראי על אינדוקציה ותחזוקה של פסוריאזיס11.

רשתות ציטוקינים מורכבות תוארו בפסוריאזיס, והתפקיד המרכזי של ציטוקינים פרו-דלקתיים (כגון IL-23, IL-22, IL-17, IL-1α, oncostatin M(OSM) ו-TNF-α) המיוצרים על ידי חדירת תאי חיסון הודגש12,13. ואכן, מחקרים קודמים הראו כי רמות מוגברות של IL-17A, IL-22, IL-1α, TNF-α ו-OSM גרמו לפרופיל של פסוריאזי על קרטינוציטים אפידרמיסיים אנושיים רגילים במבחנה14.

קווי תאי קרטינוציטים בני אלמוות (HaCaT) הניתנים להשגה ולתרבית בקלות רבה יותר מאשר קרטינוציטים ראשוניים עם יכולת שחזור טובה יותר, היו בשימוש נרחב לחקר פסוריאזיס 15,16,17,18,19,20. בשונה מנגיף הפפילומה האנושי16 E6/E7 שהפך לתאי HEK001 ו-KerTr, קו תאי HaCaT מסוגל לבטא תוצרי גן התמיינות, כולל Keratin1 (KRT1), Keratin10 (KRT10), Loricrin, ו-filaggrin 20,21,22, ובכך מספק כלי מבטיח הדומה לקרטינוציטים ראשוניים לחקר ויסות הקרטיניזציה והפרו-דלקתי.

בסך הכל, מטרתנו הייתה ליצור מודל קרטינוציטים דלקתיים בתאי HaCaT באמצעות שילוב ציטוקינים שיוכל לסכם באופן סינרגטי כמה מאפיינים של נגעי עור פסוריאזיס, כולל התחלת תגובה חיסונית, התפשטות קרטינוציטים והתמיינות.

פרוטוקול

בצע את שלבים 1 עד 3 בתנאי סטרילי. כל מדיום התרבית הכיל 0.1 מ"ג/מ"ל פניצילין וסטרפטומיצין.

1. הכנת תאים

  1. זרעים 1 x 106 תאי HaCaT ב-10 מ"ל של מדיום הנשר המותאם (DMEM) של Dulbecco בתוספת 10% סרום בקר עוברי (FBS) בצלחת תרבית תאים 100 מ"מ. דגרו את צלחת התרבות בטמפרטורה של 37 מעלות צלזיוס בחממה לחה של 5% CO2 למשך יומיים.
  2. כאשר התרבית מגיעה לסביבות 80% מפגש, הסר בזהירות את המדיום מכלי התרבות ושטוף את התאים עם 5 מ"ל של מי מלח חוצץ פוספט 1x (PBS). נדנד בעדינות את צלחת התרבות באופן ידני.
  3. הסר PBS והוסף 2 מ"ל של תמיסת טריפסין-EDTA של 0.25% לתאים. נענע בעדינות את צלחת התרבות כדי לתת לתמיסה לצפות לחלוטין את שכבת התאים החד-שכבתית.
  4. דגרו את התאים בטמפרטורה של 37 מעלות צלזיוס באינקובטור לח של 5% CO2 למשך 5 דקות עד שהתאים מנותקים באופן גלוי מפני השטח של צלחת התרבות במיקרוסקופ ניגודיות פאזה.
    הערה: המורפולוגיה של התאים צריכה להופיע עגולה. אם התאים אינם מנותקים היטב, יש לדגור לתקופה נוספת של 5 דקות יחד עם תסיסה ידנית.
  5. לאחר ניתוק התאים, הוסף 5 מ"ל של DMEM בתוספת 10% FBS כדי לנטרל את הטריפסין ולאסוף את תרחיף התאים בצינור צנטריפוגה של 50 מ"ל באמצעות פיפטה.
  6. צנטריפוגה הצינור המכיל תרחיף תאים ב-180 x גרם למשך 5 דקות. לשפוך את הסופרנטנט.
  7. הוסף 10 מ"ל של DMEM בתוספת 10% מדיום FBS לגלולה ופיפטה בעדינות את המדיום למעלה ולמטה כדי להביא את התאים לתרחיף.

2. זריעת תאים בצלחת 6 בארות

  1. זרע את התאים בצפיפות של 2.0 x 105 תאים / באר בצלחת תרבית של 6 בארות.
  2. דגרו את צלחת התרבית בת 6 הבארות ב-37 מעלות צלזיוס באינקובטור לח של 5% CO2 למשך הלילה לפני גירוי משולב M5 כדי לאפשר לתאים להיצמד לצלחת.

3. גירוי M5 של תאי HaCaT

  1. הכן מדיום תערובת שילוב ציטוקינים M5 המכיל 10 ננוגרם/מ"ל של חלבון רקומביננטי IL-17A, IL-22, IL-1α, TNF-α ואונקוסטטין M ב-DMEM המכיל 2% FBS.
    הערה: שילוב M5 כלל רקומבינציה של IL-17A, IL-22, IL-1α, TNF-α ואונקוסטטין M. פעולה סינרגטית של שילוב M5 מסכמת כמה מאפיינים של פסוריאזיס, כמו ויסות מוגבר של כימוקינים וייצור פפטידים אנטי-מיקרוביאליים31.
  2. לאחר תרבית תאים למשך הלילה, הסר את הסופרנטנט, ופיפטה 2 מ"ל של שילוב M5 מערבבים בינוני לכל באר.
  3. המשך בתרבית התאים; ליזטים של תאים נאספו לאחר 24 שעות לכימות mRNA (שלב 4) או תרבית סופרנטנט נאספו לאחר 48-72 שעות לקביעת רמות הציטוקינים על ידי בדיקת ELISA 32,33,34,35,36,37 (שלב 6).

4. קציר mRNA של תאי HaCaT מגורים M5

  1. שאפו את תערובת השילוב M5 בינונית ושטפו פעם אחת עם 1-2 מ"ל PBS קר.
  2. שאפו PBS והוסיפו 1 מ"ל של תמיסת גואנידיום איזותיוציאנט הזמינה מסחרית ישירות לצלחת התרבית כדי ליז את התאים.
  3. פיפטה את הליזאט למעלה ולמטה מספר פעמים כדי להומוגניזציה ולהעביר את הליזט של התא לצינור מיקרו-צנטריפוגה של 1.5 מ"ל. השאירו בטמפרטורת החדר למשך 5 דקות.
  4. מוסיפים 200 מיקרוליטר כלורופורם, מנערים את הצינור במרץ במשך כ-20 שניות ודוגרים את הדגימה למשך 5 דקות בטמפרטורת החדר.
  5. צנטריפוגה ב-12,000 x גרם למשך 10 דקות ב-4 מעלות צלזיוס.
  6. העבירו את השלב המימי לצינור מיקרו-צנטריפוגה טרי של 1.5 מ"ל.
  7. מוסיפים 500 מיקרוליטר איזופרופנול לשלב המימי ומערבבים בעדינות. השאירו בטמפרטורת החדר למשך 5 דקות.
  8. צנטריפוגה ב-10,000 x גרם למשך 15 דקות ב-4 מעלות צלזיוס.
  9. שאפו איזופרופנול והוסיפו 1 מ"ל של 75% אתנול. שוטפים את הגלולה פעם אחת.
  10. צנטריפוגה ב-7,500 x גרם למשך 5 דקות ב-4 מעלות צלזיוס. יוצקים את האתנול ונותנים לכדורים להתייבש באוויר.
  11. הוסף 30 מיקרוליטר מים מטופלים DEPC לגלולת ה-RNA. התכוננו לזיהוי RT-PCR.

5. ניתוח ביטוי mRNA על ידי PCR בזמן אמת

  1. בצע סינתזת cDNA עם 1 מיקרוגרם של RNA כולל באמצעות ערכה זמינה מסחרית בהתאם להוראות היצרן.
    הערה: 1 מיקרוגרם של RNA כולל עבור בדיקת SYBR Green RT-PCR.
  2. הכן את תערובת תגובת ה-PCR. 12.5 מיקרוליטר של פרמיקס SYBR EX Taq II, 1 מיקרוליטר של פריימר PCR קדימה (10 מיקרומטר), 1 מיקרוליטר של פריימר הפוך PCR (10 מיקרומטר), 2 מיקרוגרם של cDNA ו-8.5 מיקרוליטר של dH2O. הנפח הסופי הוא 25 מיקרוליטר לתגובה.

  3. דגירה במחזור תרמי. תנאי הרכיבה כללו מדרגת דנטורציה ב-95 מעלות צלזיוס למשך 30 שניות, 40 מחזורים של 95 מעלות צלזיוס למשך 5 שניות, 60 מעלות צלזיוס למשך 30 שניות, 72 מעלות צלזיוס למשך 20 שניות וניתוח עקומת התכה.
  4. נתח נתוני ניסוי RT-PCR על ידי חישוב הבדלים יחסיים בביטוי גנים מערכי Ct (מחזור סף) באמצעות משוואות 2-ΔΔCT. השתמש ב-β-actin כבקרה פנימית לסטנדרטיזציה של ביטוי גנים כמותי יחסי של RT-PCR.

6. קצירת תרבית תאים עבור ELISA

  1. הכניסו כל אמצעי תרבית תאים לתוך צינור מיקרו-צנטריפוגה של 1.5 מ"ל.
  2. צנטריפוגה ב-1500 x גרם למשך 10 דקות ב-4 מעלות צלזיוס.
  3. יש לצרוך את הסופרנטנט ולאחסן מיד בטמפרטורה של -80 מעלות צלזיוס.

7. ניתוח ביטוי ציטוקינים על ידי ELISA

  1. עקוב אחר הוראות היצרן כדי להכין את כל הריאגנטים, תקן העבודה והדגימות. דילול טורי פי שלושה של תקן עם דילול דגימה שנע בין 2,000 ל-2.74 pg/mL. דגימות מדללות 1:2 עם מדלל דגימה.
  2. הוסף 100 מיקרוליטר של תקן ודגימה לבאר. מכסים בארות במכסה ואוטמים את הצלחת בניילון פרפין. דגירה למשך 2.5 שעות בטמפרטורת החדר.
  3. לאחר 2.5 שעות, השלך את התמיסה. הוסף 300 מיקרוליטר של מאגר כביסה לכל באר בצלחת למשך 3 דקות ושואב. חזור על התהליך ארבע פעמים נוספות. לאחר הכביסה האחרונה, הפוך את הצלחת והקש על נייר סופג כדי להסיר את הנוזלים שנותרו.
  4. הוסף 100 מיקרוליטר מתמיסת נוגדני הזיהוי לכל באר. דגרו את הצלחת על שייקר למשך שעתיים בחום של 37 מעלות צלזיוס.
    הערה: תמיסת נוגדנים לזיהוי ביוטיניל מוכנה בכל ערכות ELISA.
  5. שאפו ושטפו כל באר צלחת חמש פעמים באמצעות 300 מיקרוליטר של מאגר כביסה. לאחר הכביסה האחרונה, הפוך את הצלחת והקש על נייר סופג כדי להסיר את הנוזל שנותר.
  6. הוסף 100 מיקרוליטר של מצומד HRP-Streptavidin לכל באר. יש לדגור במשך 45 דקות בטמפרטורת החדר עם ניעור עדין.
  7. השליכו את התמיסה על ידי שאיבה עם פיפטה לאחר הדגירה. שטפו כל באר צלחת חמש פעמים באמצעות 300 מיקרוליטר של מאגר כביסה. לאחר הכביסה האחרונה, הפוך את הצלחת והקש על הנייר הסופג כדי להסיר את הנוזל שנותר.
  8. בצע גילוי על ידי הוספת 100 מיקרוליטר של מצע TMB לכל באר. יש לדגור במשך 30 דקות בטמפרטורה של 37 מעלות צלזיוס בחושך.
  9. הוסף 50 מיקרוליטר של תמיסת עצירה, כאשר מתפתח צבע, לכל באר. הקש בעדינות על הצלחת כדי להבטיח ערבוב יסודי. קרא מיד ב -450 ננומטר.

תוצאות

גירוי משולב M5 גרם לתגובה דלקתית של תאי HaCaT.
תאי HaCaT עוררו עם או בלי שילוב ציטוקינים M5 במשך 24 שעות. הוערך ביטוי mRNA של גנים הקשורים לפסוריאזיס, המעורבים בוויסות הכימוקינים החיסוניים והדלקתיים והפפטידים האנטי-מיקרוביאליים. כימוקינים נויטרופילים CXCL138, CXCL239, CXCL840 וכימוקינים של תאי T CCL20 41,42
