מבוא

הצגת מקרה
גבר בן 49, שחקן ג'ודו חצי מקצועי לשעבר עם ציון ISI13 10 מ-5, הציג חוסר יציבות חוזר בכתף ימין לאחר נקע בפעם הראשונה שלו ב-2006 לאחר נפילה במדרגות. הוא עבר שני תיקונים ארתרוסקופיים כושלים בבנקרט ב-2006 ו-2007 עם עמדת עוגן לא מושלמת. בשל המצב הלא פתור, הוא עבר הליך Latarjet פתוח במרץ 2020. איור 1 הדגים את צילום הרנטגן לפני ואחרי הניתוח לאחר הליך Latarjet. בינואר 2023, המטופל חווה נפילה נוספת ממספר מדרגות, וכתוצאה מכך שוב נקע של כתף ימין.

אבחון, הערכה ותכנון

פרוטוקול

מחקר זה אושר על ידי ועדת האתיקה של המוסד שלנו והתקבלה הסכמה מדעת מהמטופל.

  1. שים את המטופל בתנוחת כיסא חוף עם מחזיק זרוע בהרדמה כללית וחסימת עצב בין-סקלנית.
  2. צור פורטלים אחוריים, אחוריים, קדמיים ואנטרולטרליים עם להב מס' 11 (איור 3A).
  3. בצע בדיקה ארתרוסקופית יסודית עם ארתרוסקופ של 4 מ"מ בזווית של 30 מעלות כדי להגדיר את מיקום נגע היל-זקס מהשער הקדמי.
  4. נקה את הבורסה התת-אקרומיאלית עם להבי גילוח 5.5 מ"מ. העבירו את הארתרוסקופ דרך הפורטל האנטרולטרלי כדי להעריך את גודל נגע היל-זקס תוך מפרקי (איור 3B).
  5. הסירו את מצע עצם הזרוע הפרוקסימלי החשוף (איור 3C).

2. שיקום

  1. לשתק את הזרוע המנותחת עם מתלה למשך 2-3 השבועות הראשונים.
  2. הנחו את המטופל לחדש בהדרגה את הפעילות כל עוד הם אינם מעוררים כאב.
  3. למד את המטופל על תרגילי שיקום עצמי ביתיים מאז היום הראשון שלאחר הניתוח, ואחריו הפרוטוקול שתוכנן על ידי רופא ספורט15.
  4. לאפשר חזרה לספורט ותרגילי חיזוק על ידי פיזיותרפיסט מנוסה 3 חודשים לאחר הניתוח.

תוצאות

שנה לאחר הניתוח, התוצאות המדווחות של המטופל הראו שיפורים משמעותיים. ציון חוסר יציבות הכתף של אוקספורד השתפר מ-26 ל-54, ציון QuickDASH ירד מ-63.6 ל-11.4, וציון ה-WOSI ירד מ-78.4 ל-15.1. צילומי רנטגן של שנה לאחר הניתוח הראו שילוב מלא של שתל העצם ללא עדות לאוסטאוליזה (איור 5A). סריקת טומוגרפיה ממוחשבת בוצעה 12 חודשים לאחר הניתוח כדי להעריך את איחוד העצם בין השתל לעצם המארח (איור 5B).

קבלת החלטות בעקבות הליך כושל של Latarjet
