תקציר

מחקר זה ניתח באופן שיטתי 1,197 מאמרים על תפקידם של מקרופאגים בזאבת אריתמטית מערכתית (SLE) שפורסמו בין השנים 2016 ל-2025 באמצעות שיטות ביבליומטריות המבוססות על אוסף הליבה של Web of Science. הניתוח בחן מגמות פרסום, רשתות שיתופיות, נושאי מחקר ומאגר ידע כדי להבהיר את נוף המחקר הנוכחי, כוחות הליבה של המחקר והתפתחות הגבול בתחום זה. מספר הפרסומים הגיע לשיא של 154 בשנת 2022, בעוד שמספר הציטוטים השנתיים עלה מ-184 בשנת 2016 ל-9,030 בשנת 2025, מה שמעיד על מחקר פעיל ומשפיע. סין דורגה ראשונה בנפח הפרסום (379 מאמרים), בעוד שארצות הברית הובילה בסך הציטוטים (18,838) ובמרכזיות שיתוף הפעולה הבינלאומי. אוניברסיטת שנחאי ג'יאו טונג ומכון קרולינסקה היו המוסדות הפוריים ביותר, עם 23 מאמרים כל אחד. חזיתות באימונולוגיה (מדד השפעה = 5.9; רבעון 1 של דוחות ציטוט בכתב העת היה הפורה ביותר. קיבוץ ציר הזמן של מילות מפתח זיהה 13 נושאים מרכזיים מחקריים, כולל רכיבים מרכזיים של חיסון מולד כגון מלכודות חוץ-תאיות של נויטרופילים, תאי דנדריט וקולטנים דמויי טול, וכן חסינות אדפטיבית הכוללת תאי T, תאי B ותאי דנדריטיים פלזמציטואידיים. נושאים אלו כללו גם מנגנונים מרכזיים כמו קיטוב מקרופאגים, אינפלמזום NLRP3 (תחום פירין 3) המכיל את משפחת הקולטנים דמוית NOD, ופינוי תאים אפופטוטיים, לצד מוקדי חום קליניים כמו דלקת זאבת, תסמונת הפעלת מקרופאג' ודלקת מפרקים שגרונית. התפתחות מוקדי המחקר משקפת התקדמות ממנגנונים בסיסיים מוקדמים של ניקוי תאים אפופטוטיים ומורכבים חיסוניים לוויסות משופר של ציטוקינים, חיסון מולד וחיסנות אדפטיבית, ולבסוף למחקרים של טיפול ממוקד מסלולי איתות, ניהול קליני ואימות רב-מרכזי, כאשר "מסלול" ו"אבחון רב-מרכזי" בולטים כחזיתות מחקר עכשוויות.

מבוא

לופוס אריתמטוז מערכתית (SLE) היא מחלה אוטואימונית כרונית המאופיינת בייצור נוגדנים עצמיים, הצטברות קומפלקס חיסוני ודלקת רב-איברית 1,2. למרות התקדמות משמעותית בגישות אבחון ואסטרטגיות טיפוליות בעשורים האחרונים, SLE ממשיכה לגרום לנזק חמור לכליות, למערכת ההמטולוגית ולמערכת העצבים המרכזית 3,4,5. טיפולים קיימים, כולל גלוקוקורטיקואידים, מדכאי חיסון וסוכנים ביולוגיים, בדרך כלל בעלי יעילות טיפולית מוגבלת וקשורים לתופעות לוואי בדרגות משתנות 6,7,8. אסטרטגיות הנדסת תאים, כגון טיפול בתאי קולטני אנטיגן כימרי (CAR-T), הראו פוטנציאל להשיג הפוגה עמידה ללא תרופות בזולגת (SLE) באמצעות התרוקנות עמוקה של תאי B אוטוריאקטיביים 9,10,11. כחברים מרכזיים במערכת החיסון המולדת, המאקרופאגים ממלאים תפקידים חיוניים בניקוי קומפלקסים חיסוניים, אפרוזיטוזיס של תאים אפופטוטיים והפרשת ציטוקינים, ובכך שומרים על הומאוסטזיס חיסוני11,12. הראיות המצטברות מצביעות על כך שמספר צורות של תפקוד לקוי של מקרופאגים מעורבים בפתוגנזה של SLE 13,14,15. קיטוב לא מבוקר בין מקרופאגים ל-M1 פרודלקתי לעומת M2 אנטי-דלקתי נצפתה בעקביות בדלקת זאבת, כאשר מקרופאגים M1 מקדמים נזק לרקמה בעוד שמקרופאגים M2 תורמים לתיקון רקמות וכן לעיצוב פיברוטי16,17. היווצרות מופרזת של מלכודות חוץ-תאיות (NETs) של נויטרופילים והפעלה חריגה של מסלולי איתות האינטרפרונים מגבירים עוד יותר תגובות דלקתיות המתווכות על ידי מקרופאגים 18,19,20. כתוצאה מכך, תפקוד לקוי של המקרופאגים לא רק הפך לצומת מרכזי בפתוגנזה של SLE אלא גם כמטרה טיפולית מבטיחה21.

בעשור האחרון נרשמה התרחבות מהירה של פרסומים המתמקדים בתפקיד המקרופאגים ב-SLE, המשתרעים על כיווני מחקר מגוונים, כולל אפרוסיטוזיס, NETosis, קיטוב מקרופאג'ים, תכנות מחדש מטבולי ומודולציה ממוקדת של מסלולי איתות 22,23,24,25,26. עם זאת, הספרות המתפתחת מביאה איתה גם אתגרים, כולל נושאים מחקריים מפוצלים, זיהוי לא ברור של כוחות מחקר מרכזיים, ומיפוי מעורפל של התפתחות הגבולות, המדגיש את הצורך הדחוף בסקירה שיטתית של נוף המחקר הנוכחי. ביבליומטריקה עושה שימוש בשיטות מתמטיות וסטטיסטיות לניתוח כמותי של מאפייני התפלגות ודפוסי אבולוציהן של נושאי ידע, ומספק נתונים אובייקטיביים על מגמות פרסום, רשתות שיתופיות, מוקדי מחקר ודינמיקת חזית 27,28,29,30,31. בעוד שגישות ביבליומטריות יושמו בהצלחה במיפוי ידע באימונותרפיה, גילוי סמנים ביולוגיים ותחומים קשורים, מיפוי ביבליומטרי המתמקד במיוחד במקרופאגים בזולה נותר מוגבל 32,33,34. מחקר זה שולף ספרות רלוונטית ממסד הנתונים Web of Science Core Collection המשתרע בין 2016 ל-2025 ומשתמש ב-CiteSpace, VOSviewer ו-Scimago Graphica לביצוע ניתוחים חזותיים רב-ממדיים. על ידי בחינת תוצרי פרסום, שיתופי פעולה בין מדינות/אזור ומוסדות, רשתות מחברים וכתבי עת, הופעה משותפת של מילות מפתח ואשכול צירי זמן, וכן זיהוי פרצי ציטוטים, מחקר זה שואף להבהיר באופן שיטתי את מצב המחקר הנוכחי, את כוחות הליבה המחקריים, את ההתפתחות התמטית ואת הגבולות המתפתחים בתחום המאקרופאגים ב-SLE, ובכך לספק מפת ידע אובייקטיבית והפניה כיוונית לחקירות עתידיות.

פרוטוקול

רכישת נתונים ואסטרטגיית חיפוש

הנתונים נאספו ממאגר Web of Science Core Collection (WOSCC) בתאריך אחד (27 באפריל 2026) כדי להבטיח עקביות ושחזור נתונים. שאילתת החיפוש השתמשה באופרטורים בוליאניים עם המבנה הבא: TS=("Lupus Erythematosus, Systemic" או "Lupus Erythematosus" או "Lupus Erythematosus Disseminatus") ו-TS=("מקרופאג'*"). סוגי המסמכים הוגבלו למאמרים ולמאמרי ביקורת. תקופת הפרסום נעה מ-1 בינואר 2016 ועד 31 בדצמבר 2025. בסך הכול נשמרו 1,197 מאמרים תקפים לניתוח ביבליומטרי מאוחר יותר.

כלים אנליטיים

נעשה שימוש בצינור אנליטי הכולל מספר חבילות תוכנה. CiteSpace (מזהה משאבי מחקר [RRID]: SCR_020932) שימש לאכולות ציר זמן של מילות מפתח ולניתוח פרץ ציטוטים35. VOSviewer (RRID: SCR_016598) שימש לבניית והמחשת רשתות מחברים משותפים למדינות/אזורים, מוסדות ומחברים, וליצירת מפות מפות משותף של מילות מפתח36. Scimago Graphica שימשה ליצירת ויזואליזציות רשת לשיתוף פעולה בין מדינות/אזור מקבצי רשת שיוצאו מ-VOSviewer37. חישובים סטטיסטיים בסיסיים והכנת איורים בוצעו באמצעות WPS Excel ו-GraphPad Prism (RRID: SCR_002798). תהליך העבודה הביבליומטרי הכולל מוצג באיור 1.

תצורת פרמטרים ב-CiteSpace וב-VOSviewer

ב-CiteSpace, פרק הזמן נקבע בין 2016 ל-2025 עם מרווחים של שנה אחת. מדד g (k = 10) נבחר כסף בחירת הצמת. אלגוריתם הגיזום Pathfinder יושם פעמיים: תחילה כדי לפשט כל רשת פרוסות זמן בנפרד, ופעם נוספת לאחר מיזוג כל הפרוסות כדי להפחית את המורכבות הוויזואלית. ב-VOSviewer נבחר אלגוריתם LinLog/מודולריות לאופטימיזציה של פריסת רשת. גודל הצמתים שוקל לפי ספירת מסמכים או תדירות ציטוטים, בהתאם לניתוח הספציפי. במיפוי שיתוף פעולה בין מדינות לאזור באמצעות Scimago Graphica, קוטר הצמתים שיקף נפחי פרסום, ועובי הקישורים ייצג עוצמות שיתוף פעולה בין מדינות.

תוצאות

תפוקת הפרסום השנתית

בין השנים 2016 ל-2025 פורסמו בסך הכול 1,197 מאמרים על מקרופאגים ב-SLE. נפח הפרסומים השנתי השתנה, החל מ-112 ב-2016, הגיע לשיא של 154 ב-2022, ואז ירד מעט ל-133 ב-2025. באותה תקופה, מספר הציטוטים השנתיים עלה בסך הכל מ-184 ל-9,030, עם תנודות קלות, המצביעות על עלייה הדרגתית בהשפעה האקדמית בתחום זה (איור 2).
