Treatment Plan Modification

Treatment plan modification is the systematic adjustment of a patient’s planned clinical care when symptoms, treatment response, risks, preferences, or diagnostic information change. Clinicians reassess the patient’s condition and goals, review progress and potential adverse effects, and then revise interventions, monitoring, or follow-up while documenting the rationale and obtaining appropriate agreement. This process supports individualized care across medical, surgical, behavioral, and rehabilitative settings. Timely modification can address inadequate response, improve treatment safety, incorporate new clinical evidence, and align care with changing patient needs, making it an essential component of ongoing clinical decision-making.
