Negative Pressure Wound Therapy

Negative pressure wound therapy (NPWT) is a wound-management technique that uses controlled suction to support healing in selected acute, chronic, and surgical wounds. A sealed dressing connects the wound to a pump, which applies subatmospheric pressure to remove exudate, reduce tissue edema, and help draw wound edges together; mechanical forces may also stimulate granulation tissue formation and local perfusion. Clinicians use NPWT to manage complex wounds, prepare wound beds for closure or grafting, and protect some surgical incisions. Its effectiveness depends on appropriate patient selection, wound assessment, dressing technique, and monitoring for complications.

We describe a "gold standard" for evaluating orthostatic tolerance (OT) using tilt testing with combined lower body negative pressure (LBNP). This can be combined with non-invasive evaluations of cardiovascular reflex control. Normal and abnormal responses are defined.

Assessing Cerebral Autoregulation via Oscillatory Lower Body Negative Pressure and Projection Pursuit Regression

Cerebral perfusion is maintained across a range of pressures via cerebral autoregulation. However, characterizing autoregulation requires prominent pressure fluctuations at regulated frequencies. The described protocol will show how oscillatory lower body negative pressure can generate pressure fluctuations to provide data for projection pursuit regression for quantification of the autoregulatory curve.
