Abdominal Exam II: Percussion

1. Percussione generale dell'addome
<ol><li>Spiegare la procedura al paziente</li>

</ol>3. Percuss toracico anteriore sinistro per rilevare la splenomegalia
La milza si trova leggermente posteriore alla linea medio-ascellare sinistra e produce un'area ovale di ottusità tra la9a e l'11a costola. Solo una piccola superficie di una milza normale è abbastanza superficiale da essere rilevata e l'ottusità splenica è spesso oscurata dal timmo gastrico o del colon. La percussione medica si basa sulla differenza di tono tra i suoni suscitati toccando la parete del corpo. La risposta uditiva al tapping dipende dalla facilità con cui vibra la parete del corpo ed è influenzata dagli organi sottostanti, dalla forza del colpo e dallo stato della parete corporea. Ci sono tre principali suoni di percussioni mediche: risonanza (ascoltata sui polmoni), timonica (ascoltata sopra i loop intestinali pieni d'aria) e ottusità (ascoltata su organi fluidi o solidi). Il contrasto tra ottusità e timosanica o risonanza consente di determinare le dimensioni e i margini di organi e masse, nonché di identificare l'accumulo di liquidi e le aree di consolidamento. La percussione rimane una parte intricata della diagnosi fisica da quando è stata introdotta per la prima volta più di 200 anni fa ed è particolarmente utile nell'esame dei polmoni e dell'addome.
Come parte di un esame addominale, la percussione segue l'ispezione visiva e l'auscultazione. L'esaminatore dovrebbe prima percuss su ciascuna delle nove regioni addominali (regione epigastrica, regione ipocondriaca destra, regione ipocondriaca sinistra, regione ombelicale, regione lombare destra, regione lombare sinistra, regione ipogastrica, regione inguinale destra e regione inguinale sinistra). La tenerezza suscitata dalle percussioni è anormale e si dovrebbe sospettare un'infiammazione peritoneale. Poiché le anse intestinali piene d'aria sono posizionate nelle immediate vicinanze della parete addominale, la percussione sulla maggior parte delle parti della cavità addominale suscita un suono prevalentemente timpanico. Abdominal percussion is a physical exam technique that uses tapping to find differences in sound. The pitch changes because the body wall vibrates differently depending on the organs underneath, the strength of the tap, and the condition of the body wall. Doctors use this method to help judge organ size, find masses, and look for fluid or areas of tissue consolidation.

Three main percussion sounds are heard in medical exam. Resonance is heard over the lungs. Tympany is heard over air-filled bowel loops, and dullness is heard over fluid or solid organs. The contrast between dullness and the more hollow sounds helps the examiner define organ margins and detect abnormal areas.

During an abdominal exam, percussion comes after visual inspection and auscultation, which means listening with a stethoscope. The examiner first percusses the nine abdominal regions: epigastric, right hypochondriac, left hypochondriac, umbilical, right lumbar, left lumbar, hypogastric, right inguinal, and left inguinal. Tenderness with percussion is not normal and can suggest peritoneal inflammation.

Most of the abdomen normally gives a tympanic sound because air-filled bowel loops lie close to the abdominal wall. Large areas of dullness should raise concern for organ enlargement, intra-abdominal masses, or fluid. The exam should also estimate liver and spleen size by percussion, including over the lower anterior chest. Normally, dullness is heard over the right anterior chest where the liver lies, while the left anterior chest is usually tympanic over the gastric air bubble and the splenic flexure of the colon.
