Accessory muscles help the respiratory system expand the chest and move air when breathing needs extra effort. The diaphragm is still the main breathing muscle, but other muscles can assist during inhalation and exhalation. This support becomes important during deep breathing or when the body faces breathing challenges.

Several accessory muscles help with inhalation. The sternocleidomastoid, which is in the neck, lifts the sternum and helps the chest cavity expand. The scalene muscles also sit in the neck and raise the second and third ribs. The pectoralis minor, found in the upper chest and front of the shoulders, elevates the third, fourth, and fifth ribs. Together, these muscles increase lung expansion and help more oxygen reach the bloodstream.

Accessory muscles also assist with exhalation, especially when air must leave the lungs more forcefully. During normal exhalation, the diaphragm and intercostal muscles relax, and the chest cavity becomes smaller. During forced exhalation, such as while exercising hard or coughing, the abdominal muscles contract and push up against the diaphragm. This action helps expel air from the lungs more strongly.
