L'hazard ratio confronta il pericolo o il rischio che un evento si verifichi in un dato momento tra due gruppi.
Consideriamo λ(t) come un tasso di rischio al tempo t, che misura la velocità con cui una percentuale di una popolazione sopravviverà fino a quando il tempo t non diminuirà in quel momento.
In altre parole, può essere la proporzione di persone che moriranno tra quelle che sono in vita fino al momento t.
Ora, i tassi di rischio per il gruppo di controllo A e il gruppo sperimentale B possono essere espressi rispettivamente come λA(t) e λB(t). Quindi, il rapporto tra λB(t) e λA(t) è l'hazard ratio indicato come θ(t).
Un hazard ratio inferiore a 1 suggerisce che un evento come la morte è meno probabile nel gruppo sperimentale che favorisce il trattamento sperimentale.
Un hazard ratio superiore a 1 indica una maggiore probabilità di morte nel gruppo sperimentale che favorisce il trattamento di controllo.
Infine, l'hazard ratio 1 indica la stessa probabilità di morte sia per i trattamenti di controllo che per quelli sperimentali.

Il rapporto di rischio, conosciuto anche come Hazard Ratio ed indicato con HR, è una misura ampiamente utilizzata negli studi clinici per confrontare il rischio che si verifichi un evento, come morte o recidiva della malattia, tra due gruppi nel tempo. Esso riflette il rapporto dei tassi di rischio (hazard), ovvero il rischio istantaneo che si verifichi l'evento, tra un gruppo di trattamento e un gruppo di controllo. Questa misura fornisce informazioni preziose sull'efficacia relativa di un trattamento valutando come il rischio di un evento differisce tra i due gruppi.

Ad esempio, in uno studio clinico che valuta un nuovo farmaco antitumorale rispetto alla terapia standard, il rapporto di rischio quantifica l'impatto del farmaco su risultati come recidiva del cancro o sopravvivenza. Un rapporto di rischio inferiore a 1 suggerisce che il gruppo di trattamento ha un rischio inferiore di evento, indicando che il nuovo trattamento potrebbe essere utile. Al contrario, un HR superiore a 1 implica un rischio più elevato nel gruppo di trattamento, favorendo l'intervento di controllo. Un rapporto di rischio esattamente pari a 1 indica che non vi è alcuna differenza di rischio tra i gruppi.

Nella ricerca clinica, il rapporto di rischio è particolarmente prezioso perché considera la tempistica degli eventi, non solo il loro verificarsi entro la fine di uno studio. A hazard ratio below 1 means the treatment group has a lower risk of the event. That result suggests the new treatment may help. A hazard ratio above 1 means the treatment group has a higher risk, which favors the control intervention. A hazard ratio of 1 means there is no difference in risk between the groups.
