The actuarial method calculates survival rates at predetermined intervals, fully utilizing data from all participants, including those lost to follow-up or who have died from other causes.
To understand the impact on first-year survival, one-year survival rates are derived by subtracting the number of deaths from the total participants and dividing by the original number.
Only the subset of patients observed for the full two years is considered for estimating two-year survival, focusing on those who survived the first year and then continued to survive the second year.
This method is consistent, helps identify long-term trends, and evaluates the effectiveness of treatments or interventions, providing a robust tool for understanding outcomes in medical research.

The actuarial approach, a statistical method originally developed for life insurance risk assessment, is widely used to calculate survival rates in clinical and population studies. This method accounts for participants lost to follow-up or those who die from causes unrelated to the study, ensuring a more accurate representation of survival probabilities.
Consider the example of a high-risk surgical procedure with significant early-stage mortality. A two-year clinical study is conducted, focusing on the critical first year. Participants are divided into two groups: one followed for a year and another for two years. Survival rates are estimated using the actuarial (or life-table) method, which divides the study period into intervals&#8212;typically one year.
To illustrate, suppose the first group starts with 1,000 patients, of whom 240 die within the first year. The second group also begins with 1,000 patients, with 200 deaths in the first year and 16 in the second year. The one-year survival rate is calculated by subtracting the total number of deaths from the initial cohort size and dividing the result by the starting population:
One-year survival rate = (2000&#8722;240&#8722;200)/2000=0.78&#8201;or&#8201;78%
This approach can also be applied to studies of long-term outcomes, such as evaluating the efficacy of a cancer treatment over years. The actuarial method accommodates patients lost to follow-up or who die from unrelated causes, allowing for a robust analysis of long-term effects.
For two-year survival rates, the calculation becomes more nuanced. It uses conditional probability, considering only those individuals observed for the full two years and who survived the first year. The two-year survival rate must not exceed the one-year rate and is determined by multiplying the probability of surviving the first year by the probability of surviving the second year, conditional on surviving the first year:

This sophisticated method, while powerful, faces challenges such as incomplete follow-up data and accurate recording of deaths. Despite these limitations, it remains particularly effective in large-scale population studies where tracking individual participants is impractical. By accounting for censoring and interval-based survival probabilities, the actuarial approach provides a reliable framework for survival analysis in diverse research contexts.

In health research, it gives a clearer picture of survival when some participants are lost to follow-up or die from causes unrelated to the study.

This method works by dividing the study period into set intervals, usually one year. That makes it useful for tracking outcomes over time, especially in studies with early deaths after a high-risk surgical procedure. In one example, a two-year study follows two groups. One group is observed for one year, and the other is observed for two years.

The one-year survival rate is found from the starting group size and the number of deaths during that year. In the example, 1,000 patients begin in each group. The first group has 240 deaths in the first year, and the second group has 200 deaths in the first year. Using the actuarial calculation, the one-year survival rate is 78%.

The same method can also be used for long-term studies, such as testing how well a cancer treatment works over several years. It accounts for patients who are not followed for the full study or who die from unrelated causes. That makes the survival estimate more realistic when individual tracking is difficult.

Two-year survival uses conditional probability, which means the second year is calculated only for people who survived the first year and were observed for the full two years. In the example, the second-year survival probability is multiplied by the first-year survival probability. The result is 0.78 times 0.98, or 76.44%.

A two-year survival rate should not be higher than the one-year rate. The method is powerful, but it depends on complete follow-up and accurate death records. Even with those limits, it is especially useful in large population studies where following each person closely is not practical.
