Change-of-shift Report

A change-of-shift report is a structured clinical handoff in which outgoing healthcare professionals transfer essential patient information and responsibility to the incoming team. It works by organizing communication around the patient’s current condition, recent changes, ongoing treatment, pending tasks, and safety concerns, while allowing questions and clarification to support accurate information exchange. Used in hospitals, clinics, and other care settings, this process promotes continuity of care, reduces omissions and misunderstandings, and helps the new team prioritize assessments and interventions. Consistent reporting practices also strengthen teamwork, situational awareness, and patient safety across transitions.
