Interdisciplinary Team

An interdisciplinary team is a group of professionals from different fields who combine their expertise to address a shared problem, particularly when no single discipline can provide a complete solution. In clinical care, team members such as physicians, nurses, pharmacists, therapists, and social workers communicate, integrate patient information, and coordinate decisions around common goals. This collaborative process supports comprehensive assessment, individualized treatment planning, continuity of care, and attention to medical, functional, and social needs. Interdisciplinary teams are especially valuable for complex or chronic conditions, where coordinated practice can improve safety, efficiency, patient experience, and health outcomes.
