Patient Care Plan

A patient care plan is a structured clinical record that organizes a patient’s assessed needs, prioritized problems, goals, and planned interventions to support safe, coordinated care. Clinicians develop it by gathering history and assessment findings, identifying relevant diagnoses, setting measurable outcomes, selecting evidence-informed interventions, and revising the plan as the patient’s condition changes. In clinical practice, the plan guides communication among healthcare professionals, helps patients participate in decisions, and provides a framework for monitoring response, documenting care, and identifying unmet needs. Its use supports individualized treatment and continuity across hospitals, outpatient clinics, and long-term care settings.

This protocol describes the use of 3D planning and printing for reconstruction of bony defects. We use segmentation tools to create 3D models followed by 3D design software to create patient specific implants for reconstruction purposes concomitant to ablative surgery or as a second stage.
