概要

急性呼吸窮迫症候群(ARDS)は、重症患者における低酸素呼吸不全および死亡の一般的な原因である1。これは、肺胞上皮および内皮損傷の両方によって特徴付けられるが、透過性および肺水腫の増加、肺胞液クリアランス(AFC)の変化、および呼吸窮迫の悪化を招く。肺胞浮腫の再吸収とARDSからの回復は、肺胞を通る上皮液輸送がそのまま残ることを必要とし、AFCを改善する治療が3,4に有用であり得ることを示唆している。肺保護換気と静脈内流体療法のための制限的な戦略は、結果2、5を改善する上で有益であることが証明されているが、彼らはまだ高い死亡率と罹患率6に関連している。したがって、症候群に対する効果的な治療法を開発し、そのような治療法が機能する可能性のある正確なメカニズムをよりよく理解することが急務です。

イオブルランやセボフルランなどのハロゲン化麻酔薬は、手術室での全身麻酔に広く使用されています。セボフルランは、胸部手術を受けている患者の肺の炎症の減少と、ARDS7などの術後肺合併症の減少に関連している。同様の結果は、心臓手術後の患者のメタ分析8で発見された。ハロゲン化揮発性物質はまた、気管支拡張効果9、10、心臓8、11および腎臓12、13、14などのいくつかの器官を保護するいくつかの特性を有する。近年、集中治療室(ICU)での鎮静剤として吸入麻酔薬の臨床利用に対する関心が高まっている。動物およびヒトの研究は共に、肝臓15、脳16、または心臓11の長期虚血の前にハロゲン化剤による前処理の保護効果を支持する。ハロゲン化剤はまた、重症患者の沈下に対する他の静脈内薬剤よりも潜在的な薬物動態学的および薬物力学的利点を有する。吸入ハロゲン化剤は、心臓手術を受けている患者における静脈内沈下と比較して挿管時間を減少させる17。いくつかの研究は、ICU患者18、19、20の沈下におけるハロゲン化剤の安全性と有効性を支持する。ARDSの実験モデルにおいて、吸入されたセボフルランはガス交換21、22を改善し、肺胞浮腫21、22を減少させ、肺および全身性炎症23の両方を減衰させる。イソフルランはまた、肺胞毛細血管バリアの完全性を維持することによって傷害後の肺修復を改善し、おそらくキータイトな接合タンパク質24、25、26の発現を調節することによって起こる。さらに、イオブルランで培養処理したマウスマクロファージは、イオブルラン27で処理されなかったマクロファージよりも好中球に対する貪食効果が優れている。

しかし、揮発性麻酔薬の肺保護特性を説明する正確な生物学的経路およびメカニズムは、現在までにほとんど知られておらず、さらに調査が必要である18。また、肺損傷に対するセボフルランの正確な影響を調査し、実験的証拠を患者に翻訳できるかどうかを検証するための追加研究も保証されています。我々のチームからの最初の無作為化対照試験は、ARDS患者における吸入セボフルランの投与が、高度な糖化末端産物の血漿および肺胞可溶性受容体(sRAGE)28によって評価されるように、炎症前のサイトカインおよび肺上皮傷害マーカーの両方の酸素化改善および減少レベルに関連していることを発見した.sRAGEは現在、肺胞型1細胞損傷および肺胞炎症の主要なメディエーターのマーカーとして考えられているので、これらの結果は、肺胞上皮損傷21、29、30に対するセボフルランの有益な効果を示唆する可能性がある。

吸入ICU沈下のためのハロゲン化剤の使用は、長い間、手術室の麻酔換気装置およびガス気化器をICUに配備する必要がある。それ以来、最新のクリティカルケア換気装置での使用に適した麻酔用反射器がICU31で特定の用途に開発されています。これらのデバイスは、呼吸回路のYピースと気管内チューブの間に挿入された変更された熱と水分交換フィルタを備えています。それらは、イソフルランおよびセボフルランが最も頻繁に使用されるハロゲン化剤の投与を可能にし、特定のシリンジポンプによって送達される液体剤が放出される多孔性ポリプロピレンエバポレーターロッドで構成される。ハロゲン化剤は、デバイスに含まれる反射媒体によって満了時に吸収され、次のひらめきの間に放出され、期限切れのハロゲン化剤31、32の約90%の再循環を可能にする。最近では、50 mLの器械デッドスペースで小型化されたバージョンが開発され、ARDS患者の超保護換気時の使用にはさらに適しており、潮量は200mL31と低くなる可能性があります。このような小型化装置はARDSの実験子豚モデルでは研究されていない。

これまでの研究ではARDS中の肺胞の炎症や損傷におけるハロゲン化揮発性物質の有望な役割を支持しているため、ハロゲン化剤が肺損傷および修復33,34,35に及ぼす影響のメカニズムを翻訳的に理解するために実験動物モデルを設計しました。本研究では、ICU型装置である麻酔薬保存装置の小型化バージョンを用いて吸入性の固虫を送達できる子豚中の塩酸(HCl)誘導ARDSのモデルを開発した。ARDSのこの大きな動物モデルは、吸入ハロゲン化剤の潜在的な肺保護効果の理解をさらに深めるために使用することができる。

プロトコル

この研究プロトコルは、preclinicaltrials.eu(臨床前のレジストリ識別子PCTETE0000129)に登録される前に、フランスのミニステア・ド・レ・エデュケーション・ナショナル、ド・レンセワニエジェネ・スペリエール・エ・デ・ラ・レシェルシュ(承認番号01505.03)の動物倫理委員会によって承認されました。すべての手順は、動物研究:インビボ実験(ARRIVE)ガイドライン36に従って、センター・インターナショナル・ド・チルジー・エンドスコピック、クレルモン・オーヴェルニュ、クレルモン・フェラン、フランスのクレルモン・フェランで行われました。

1. 動物の準備と麻酔

  1. ピグレットモード
    1. 実験プロトコルが、3Rの原則(交換、削減、改良)や国内/国際規制を含む動物実験のガイドラインと一致していることを確認してください。
    2. プロトコルを開始する前に、関連機関での実験動物のケアと使用に関する倫理委員会の承認を得てください。
    3. オスの白いランドラセの子豚(生後2~4ヶ月、体重10~15kg)を使用してください。
    4. 筋肉内アザペロンを使用して前投薬後に、子豚を腹腔の位置に置く(1.2.2に記載)。
  2. 麻酔誘導
    1. 動物が一晩食べ物を食べないように制限し、水への無料アクセスを許可する。
    2. 筋肉内アザペロン(2 mg.kg-1)を使用して、耳の後ろに抗不安前投薬を投与する。
    3. 子豚の耳介基の軟組織に指圧を加えて、内側および内側の耳介静脈を識別する。
    4. 子豚の内側または内側の耳介静脈に末梢静脈内22Gカテーテルを挿入する。皮膚を通して45°の浅い角度でカテーテルに従い、カテーテルを通して血液が現れるまで進みます。
    5. 静脈内プロポフォール(3 mg.kg-1)およびスフィタニル(0.3μ g.kg-1)37で全身麻酔を誘発する。ペダル反射への応答の欠如によって麻酔の深さを確認してください。
  3. 気管挿管38,39
    1. サイズ4ストレートミラー喉頭鏡を使用して喉頭鏡を準備します。
    2. 喉頭鏡を咽頭腔に入れ、喉頭鏡で舌を落ち込ませ、喉頭蓋炎を見えるようにする。
    3. 口蓋管挿管前の子豚の喉頭開きを可視化する。
    4. 6 mmの内径カフス気管チューブを挿入します。
    5. 気管内チューブカフを膨らませて、20〜30 cmH2Oの周りのカフ圧に達します。
    6. 気管内チューブを小子の鼻にマイクロポアの外科テープで固定します。
    7. 換気装置に接続し、セクション 3 で説明されている設定に従って機械換気を開始します。
  4. セテーションメンテナンス
    1. 酸による肺損傷の前にプロポフォール(5 mg.kg-1.h-1)の連続静脈内注入で麻酔を維持する。プロポフォールの注入は、ハロゲン化剤が開始されると停止する。
    2. 疼痛管理のために、レミエンタニルの連続静脈内注入(10-20μ g.kg−1.h−1 = 0.15-0.33μ g.kg−1.min−1)を加える。
    3. 神経筋遮断のためにシサトラキュリウム(0.2 mg.kg-1.h-1)の連続静脈内注入を加える。
    4. 暖かい毛布を使用して、子豚の体温を約38°Cに保ちます。
    5. 外部モニターを使用して、心電図活動、末梢酸素飽和度(SpO2)、および動脈圧を継続的に監視します。
  5. 手術
    1. 右の内側頸静脈の外科的暴露とセルディンガー法を用いて中央静脈アクセスを挿入し、3-ルーメンカテーテル(7フランス語、16cm)を挿入する。
      1. 首の腹側の側腹の側に、気管から2センチ側面に皮状の正中線切開を行います。外科用鉗子を使って組織を解剖する。
      2. 内部頸静脈(深さ約1〜2cm、頸動脈の横方向)を局在化し、針(18G、6.35cm)を使用して、頭蓋方向方向の穿刺を行います。
      3. 針で「J」ガイドワイヤー(直径0.81mm、60cm)を針に差し込みます。針をそっと取り出し、「J」ガイドワイヤーに沿って3本の線を持つ静脈カテーテルを内部頸静脈に素早く挿入します。静脈カテーテルを所定の位置に保ちながら、"J"ガイドワイヤーを取り外します。
      4. 静脈カテーテルの各ラインを通して血液を吸引し、異なるラインから空気を除去し、5 mLの生理食前溶液(0.9%NaCl)で洗浄し、3ラインをすすます。
      5. 連続レンバートパターンに続く3.0非吸収性縫合糸で皮膚を縫合し、中央静脈カテーテルの各側面穿光に単一のステッチとトリプルノットでカテーテルを皮膚に固定します。
    2. 右大腿動脈の外科的暴露を介して動脈線を挿入し、サーディンガー法を使用して熱希釈カテーテル(3-5フランス語、20cm)を挿入する。
      1. 子豚の右前肢を延長します。
      2. 子豚の右鼠径部に切開を行います。外科用鉗子を使って皮下組織と筋肉組織を解剖する。
      3. 大腿骨パルス(深さ約3~4cm)を触診して右大腿動脈を局在化し、針(19G、54mm)を使用して、口内方向の穿刺を行います。
      4. 針に「J」ガイドワイヤーを挿入します。針をそっと取り外し、ガイドワイヤーに沿って大腿動脈に動脈カテーテルを素早く挿入します。カテーテルを所定の位置に保ちながらガイドワイヤーを取り外します。
      5. 動脈カテーテルから空気を取り除き、ラインをすすくい食い液で洗い流します。
      6. 連続レンバートパターンに続く3.0非吸収性縫合糸で皮膚を縫合し、動脈カテーテルの各側穿点に単一のステッチとトリプルノットでカテーテルを皮膚に固定します。
      7. 動脈管にカテーテルを差し込み、連続血液サンプルの検索と、パルス輪郭心出力モニター装置を使用して連続的な血行力モニタリング(体重にインデックス化された動脈圧、心臓指数、および血管外肺水)を可能にします。

2. 酸による急性肺障害

注意:皮膚や目と酸の接触のリスクを避けるために、このステップ中に手袋や眼鏡を使用してください)

  1. 0.05 MとpH 1.4でHClの100 mLを作ります。
  2. 胸骨の最後のセグメントの解剖学的ランドマークを使用して、気管内チューブの先端と子豚のカリナとの間の距離を測定します。
  3. Ch14 吸引カテーテルの黒いペンでこの距離をマークします。
  4. 吸引カテーテルを気管内チューブから黒いランドマークまで挿入します。
  5. 吸引カテーテルを通して酸の4 mL.kg-1( 体重)を3分以上にそっと植え付けます。
  6. 吸引カテーテルを取り外します。

3. 機械換気

  1. 集中治療室の換気装置で容積制御換気を使用してください。
  2. 6 mL.kg-1の潮量、5 cmH2Oの正気の終気流圧(PEEP)、および40%のインスパイアされた酸素分率(FiO2)を使用してください。
  3. 呼吸速度を調整して、35~45mmHgの間で二酸化炭素の終潮を維持します。
    注:以前の研究に基づいて37、40、41、肺損傷は、動脈酸素緊張(PaO2)とFiO2比がベースラインから25%に低下し、気道HCl点眼後約1時間に確立すると考えられる。

4. ハロゲン麻酔薬

注:酸誘発性肺損傷が達成されると、ハロゲン化麻酔薬(セボフルランまたはイオブルラン)を使用してセドレーションを開始します。プロポフォールを使用して静脈内沈下は、その後中断する必要があります.

  1. シリンジを充填する(図1A):メーカーが提供する充填アダプタを、ハロゲン化剤の250mLボトルと60mLシリンジを充填アダプタに取り付けます。ボトルを逆さまにし、プランジャーを押したり引っ張り出したりして注射器を満たします。ボトルを直立させ、注射器を取り外します。
  2. 清掃 (図1B)
    1. 炭フィルターを置き、ハロゲン化炭化水素麻酔ガスを除去するために使用され、人工呼吸器の近くに置きます。
    2. チャコールフィルターから保護キャップを取り外します。
    3. 炭フィルターを、屈曲管で人工呼吸器の呼気弁に接続します。
  3. 麻酔用保存装置(吸入ICU沈下に使用される装置)を使用する(図1C)は、以下に説明する。
    1. 麻酔用保存装置のガスサンプリングポートにイオノマー膜乾燥機ラインを接続します。
    2. ガスサンプリングラインの片側をイオノマー膜乾燥ラインに接続します。
    3. ガスサンプリングラインの反対側をガス分析装置に接続します。
    4. 呼吸回路のY片と気管内チューブの間に麻酔用保存装置を挿入します。
    5. 麻酔用保存装置が黒い側を上にして、子豚に向かって傾斜していることを確認します。
  4. 麻酔用保存装置を介して吸入された沈着を送出する(図2)。
    1. 特定の注射器をシリンジポンプに入れる。
    2. 麻酔剤ラインをシリンジに接続します。
    3. ハロゲン化剤の1.5mLのボーラスを有する薬剤ラインをプライムする。
    4. 初期ポンプ速度をmL.h-1(イソフルランとセボフルランの初期シリンジポンプ速度設定はそれぞれ3mL/h)で、ターゲットにされた期限切れのセボフルラン画分(FEsevo)または期限切れのイソフルラン画分(FEiso)値に適応します。
    5. ガス分析器が FEsevo %-FEiso % または同等の最小肺胞濃度値をゼロより大きく表示していることを確認します。必要に応じて、ハロゲン化剤の0.3mLの追加のボーラスを与える。
    6. 分の体積と目標濃度に応じて一定の濃度に達するために必要なシリンジポンプ速度を適合させ、2~7mL.h-1および4~10 mL.h-1の割合を、一般的に、0.2%-0.7%および0.5%-1.4%の期限切れ分に関連付け
    7. 実験中に、それぞれ0.8-1.1および0.5-0.8のFEsevoおよびFEのisoターゲットを用いてハロゲン化剤の投与を続けます。

5. 測定

  1. モニタリング
    1. 外部モニターで測定されたさまざまなパラメータを収集します:心拍数、血圧、および末梢酸素飽和度。
    2. 換気装置で測定されたパラメータ:換気装置で測定されたパラメータ:潮流、呼吸数、設定PEEP、自動PEEP(人工呼吸器に5 sの呼気性保持操縦を適用する)、呼吸器のコンプライアンス、気道抵抗、吸気高原圧(人工呼吸器に2sの吸気圧を加える)、ピーク感管圧、および駆動圧力。
    3. 換気装置に組み込まれた場合は、窒素洗浄/洗浄方法を使用して、肺機能残存容量を計算します。
    4. 大腿動脈に挿入された熱指標を使用して、肺の血管外水量、心指数、全身血管抵抗を測定します。
  2. 希釈されていない肺水腫液サンプリングは、正味AFC速度を測定する。
    1. 気管内チューブを通して遠位気管支のくさび位置に柔らかい14 Fr吸引カテーテルを挿入します。
    2. 穏やかな吸引を適用することによって吸引のトラップにサンプルの浮腫の液体。

  3. ミニ気管支肺胞洗浄サンプリング。
    1. 軟らかい14 Fr吸引カテーテルを気管内チューブを通して遠位気管支のくさび位置に挿入します。
    2. 吸引カテーテルに50 mLの塩化ナトリウム溶液を入水させます。
    3. 速やかに吸引トラップに流体をサンプリングします。

      注:ミニBALの総タンパク質濃度は、比色測定法で測定され、例えば、TNF-α、IL-6、IL-1β、IL-18などの炎症性サイトカインのレベルは、多重免疫測定法を使用して測定されます。サンプルは、酸誘発性肺損傷後4時間回収される。
  4. 血液ガス分析
    1. ベースラインでBD Luer-Lokの先端と3 mL BDのBDの前置きシリンジの動脈ラインを通して動脈血気体を集める。すぐにPaO2/FiO2、PaCO2、pH、血清乳酸、および血清クレアチニンをポイントオブケア血液ガス分析装置を用いて測定する。
    2. 酸点入後4時間、このステップを1時間ごとに繰り返します。
  5. 肺サンプリング
    1. 実験終了時にペントバルビタール(150 mg.kg-1)の静脈注射で子豚を犠牲にする(酸による肺損傷後4時間)。
    2. 解剖し、肺全体を取り除く。アルコールアセチンホルマリンで修正します。
    3. パラフィンに埋め込み、厚さ10μmでスライスします。

結果

この実験では、25匹の子豚を麻酔し、2つのグループに分けました:未治療群の12匹の子豚(SHAMグループ)と酸傷群の13匹の子豚(HClグループ)。実験が終わる前に子豚は死ななかった。双方向反復測定分散分析(RM-ANOVA)は、グループ相互作用(P<10−4)によって有意な時間を示し、ArdSを持たないシャム動物と比較して、PaO 2/FiO2に対するHCl誘発ARDSの有害な影響を有する(図3)。機械的換気の4時間後に測定された全タンパク質の未希釈肺水腫レベルにおいて有意なグループ間の差が指摘された(P<10−4)。HCl誘導ARDSは、偽動物と比較した場合のBALタンパク質の増加と関連していた(図4)。双方向RM-ANOVAは、グループ相互作用(P<10-4)によって有意な時間を示し、HCl誘導ARDSは、ARDSを持たない偽動物と比較して、血管外肺水の増加に関連している(

ディスカッション

この記事では、麻酔用保存装置を用いて送達される、ハロゲン化揮発性物質(例えば、セボフルランやイオブフルラン)の肺保護効果を調べるため、子豚中のHClの気管内点滴によって誘導されるARDSの再現可能な実験モデルについて説明する。
