内視鏡超音波誘導胆道排液:悪性胆道閉塞における内視鏡超音波誘導肝胃瘻切除術

ここに提示されたのは、痛みのない黄疸と2週間の体重減少を伴う71歳の女性の場合です。コンピュータ断層撮影(CT)画像化は、リンパ節腫脹および肝転移を伴う4 x 5cmの丘陵腫瘍を示した。病変のEUS細針生検(FNB)は胆管癌と一致した。彼女のビリルビンレベルは、プレゼンテーション中に212μmol/L(<15)であった。

線形エコー内視鏡を用いて、肝臓の左拡張肝内管(IHD)の位置を特定した。セグメント3を拡張したIHDを同定し、19G針を用いて穿刺した。コントラストは、透視的ガイダンスの下でIHDsを不透明化するために使用された。IHDは0.025インチのガイドワイヤーを用いてカニューレ処理された。これに続いて、6Fr電気焼灼拡張器および4mm胆道バルーン拡張器を用いて瘻孔管を拡張した。長さ10cmの部分的に覆われた金属ステントを透視的誘導下に展開した。遠位部はIHDで開き、近位部はエコー内視鏡の作業チャネル内に展開され、その後胃に放出された。患者は処置の3日後に退院した。第2週および第4週に実施されたフォローアップは、ビリルビンレベルがそれぞれ30μmol/Lおよび14μmol/Lであることを示した。これは、EUS-HGSが切除不能な悪性胆道閉塞における胆道排液のための安全な方法であることを示している。

概要

悪性胆道閉塞を有する患者は、しばしば切除不能であり、提示時に進行する1,2。その結果、これらの症例を管理する際に、緩和的な内視鏡的胆道減圧がしばしば必要とされる3,4,5現在の勧告によると、内視鏡的逆行性胆管膵臓造影(ERCP)は、可能な限り胆道排液の主要な方法であり、これが失敗した場合または禁忌である場合、経皮経肝胆道排液(PTBD)がサルベージ方法として使用される6,7,8しかし、敗血症、胆管炎、出血、電解質喪失、漏れ、創傷感染、局所不快感を含むPTBDに関連する特定の合併症がある。これらの合併症は53.2%にも及ぶと報告されています9。内視鏡超音波誘導胆道排液(EUS-BD)の出現は、このギャップに対処するための実行可能な代替胆道排液方法を提供する。EUS-BDの主な技術は、閉塞した胆道系の治療的減圧を提供するために、胃腸管を介した胆道系へのアクセスを導くための超音波検査法の使用を含む。

EUS-BDは2001年に初めて実施され、それ以来、この胆道排水方法は長年にわたって進化してきました10。EUS-BDの方法は、一般に、EUS誘導胆道十二指腸瘻切除術(EUUS-CDS)、EUS誘導肝胃瘻切除術(EUS-HGS)、EUS誘導順行性ステント留置術(EUS-AS)、およびEUS誘導ランデブー(EUS-RV)11,12である。現在までに、EUS-BDの適応症には、ERCPに失敗した患者、十二指腸閉塞によってアクセス不能な乳頭を有する患者、および外科的解剖学的構造が変更された患者が含まれる13,14,15

EUS-HGSは、胃への左肝内管の経壁排水を含む。主な利点は、それがより生理学的であり、そして何よりもPTBD12,16と比較してより良い患者の快適さを提供する内部排液を提供することです。この方法は、丘陵性および遠位悪性胆道閉塞の両方に対して実行可能である。ここでは、切除不能な悪性丘陵腫瘍の場合におけるEUS-BDの方法の一つとしてEUS-HGSの手法について述べる。

71歳の女性は無痛黄疸を呈し、2週間で4kgの体重減少を示した。検査の結果、彼女は強膜黄疸を患っていることが判明しました。腹部検査は目立たなかった。コンピュータ断層撮影(CT)画像化は、右肝管の分岐、肝管と肝外管の合流点を含む4 x 5cmの丘陵腫瘍を示し、リンパ節腫脹および肝転移を伴う顕著な肝管拡張を引き起こした(図1A)。病変のEUS細針生検(FNB)を行い、CK7およびCA19-9陽性で腺癌を示し、これは丘陵性胆管癌の診断と一致した(図1B)。彼女のビリルビンレベルは提示時に212μmol/L(<15)であり、CA19-9レベルは305U/mLであった。EUS-HGSは、胆道閉塞の緩和を提供するために実施された。

プロトコル

プロトコルは、マラヤ大学医療センターの倫理ガイドラインに従って実施されました。書面による同意を得て、患者に手順の詳細な説明を提供しました。また、教育目的で手順のビデオを制作する許可も与えられました。

1. 位置決めと鎮静

  1. 患者を臥位に置きます。プロポフォールの静脈内注入を用いて適度な鎮静を提供する。輸液が麻酔科医によって滴定されていることを確認してください。

2. 拡張された左肝管の適切な標的の同定

  1. リニアエコー内視鏡を前進させて、胃と食道の接合部を通過することを確認します。
  2. エコー内視鏡の先端を胃腔の曲率の低い位置に置き、拡張された左肝内管(IHD)の同定に進む。
  3. 透視イメージングを行い、エコー内視鏡の位置を確認します。エコー内視鏡の先端は、前述のように、胃腔のより小さい曲率に配置する必要があります。エコー内視鏡の位置を常に維持し、手順全体を通して透視画像を使用した定期的なチェックで安定させます。
  4. 拡張セグメント3 IHDを特定する。エコー内視鏡の先端の角度をわずかに上に傾けて、針の穿刺の次のステップを容易にします。
    メモ:手順の後続のステップでは、手順全体を通してエコー内視鏡の作業チャネル を介して アクセサリを挿入し、交換します。

3.標的拡張された左肝内管への針のアクセス

  1. ドップラー超音波検査を行い、標的IHDの周囲に介在する血管がないことを確認します。
  2. 19G針を用いて、セグメント3IHDを穿刺する(図2A)。7mLの造影液をプリロードした10mLシリンジを用いて、胆汁溶液を穏やかに吸引し、これが胆道アクセスの成功を確認した。
  3. それに続いて、造影剤注入を行い、左IHDおよび胆道系の残りの部分を不透明にする。

4. ガイドワイヤー操作

  1. 0.025インチのガイドワイヤーを使用して、透視ガイダンスの下で左のIHDに移動します。
  2. ガイドワイヤーを右肝内管に横切ってカニューレをかけます(図2B)。
    注:理想的には、ガイドワイヤは総胆管の奥深くまで操作する必要があります。しかし、この場合、障害性丘陵腫瘍のために不可能であった。

5.瘻孔管拡張

  1. 4mm胆道バルーン拡張器を使用することに加えて、6Fr電気焼灼拡張器を用いて瘻孔管を拡張する(図2C)。
    注:瘻孔管拡張は、後で胆道ステントの挿入を容易にするために必要とされる。
  2. バルーンインフレーションを約5秒間行います。
    メモ:これらの作業中は、超音波画像と透視画像の両方を使用して監視対象のアクセサリの位置を監視し、ワイヤが見え、エコー内視鏡の位置が維持されていることを確認します。これは、非常に重要なアクセサリーの交換中にスムーズな移行を確実にするために行われます。

6. ステントの挿入と展開

  1. 部分的に覆われた10mmサイズの胆道ステント(長さ10cm)を透視誘導下に展開します(図3A)。
    注:ステントには、3 cmの覆われていない遠位部分と7 cmの覆われた近位部分があります。覆われていない部分は肝内管内にある必要があります(図3C)。ステントの覆われた部分の約3cmが胃腔内に残され、ステントの内方への移動を防止する(図3B)。
  2. ステントの遠位端がIHDで開いていることを確認します。近位端をエコー内視鏡の作業チャネル内に展開し、続いてステント内に流れる胆汁が見える状態で胃内に放出する。

結果

手順は約30分で完了しました。処置後に合併症はなく、患者は翌日に経口摂取を再開することができた。ビリルビンレベルは92μmol/Lに低下し、彼女は処置の3日後に退院した。繰り返しCT撮影が行われ、ステントが胆道閉塞の解像度で所定の位置にあることを示した。追跡調査のビリルビンレベルは、処置後2週間後に30μmol/L、処置後4週間後に14μmol/Lであった(表1)。

2020年6月から2021年6月の間に、切除不能な悪性胆道閉塞を有する合計15人の患者が、当院でEUS-HGSを受けた。平均年齢は65.5歳(±10.1)であり、46.7%が男性であった。膵臓がん(46.7%)が最も一般的な診断であり、外科的解剖学的構造の変化(33.3%)が処置の最も一般的な適応症であった。臨床的成功は、処置後4週間以内にビリルビンレベルが50%以上低下することと定義した。技術的成功率、臨床的成功率、および手技関連有害事象は、それぞれ86.7%、81.8%および6.7%であった。手技関連の有害事象の唯一の症例は出血であり、これは経動脈塞栓術を必要とした。1人の患者は、...

ディスカッション

上記の症例説明は、ERCPおよびPTBDなどの既存の方法と比較して、胆道悪性腫瘍の管理における代替胆道排液方法としてEUS−HGSを使用する可能性を示す。上記のステップの中で、正しい肝内管の識別とアクセス、ガイドワイヤー操作、およびステント展開は、手順を成功裏かつ安全に実行できることを保証するための3つの主要な重要なステップです。
