FITの適用は、例えば歯科用ユニットランプ1,2による従来の照明と比較して、コンポジットレジン材料と健全な歯状物質との区別を容易にする。蛍光は、材料が吸収されたよりも高い波長で光を放出するときに発生します。この照明の結果として、材料は歯3よりも明るく見える。コンポジットレジン材料の最大蛍光は、398 ± 5ナノメートル3の波長で照射されたときに発生します。コンポジットレジン材料における蛍光は、コンポジットレジン4,5の主成分であるガラスフィラーに希土類酸化物が添加されたために現れる。これらの蛍光物質の添加は、コンポジットレジンの光学特性を歯の構造に適合させ、コンポジットレジン4,5の審美性を改善することを意図している。FITは、これらの蛍光特性を示すため、多くの複合樹脂材料に適用できます3。しかしながら、蛍光は複合樹脂材料6789の経年変化とともに減少する。

歯科外傷学では、外傷スプリントを使用した負傷した歯の固定は頻繁であり、多くの場合必須です23。外傷スプリントは通常、流動性のあるコンポジットレジン材料を使用して歯に固定されます。このシナリオでコンポジットレジン材料が不完全に除去されると、上記の障害が発生する可能性があります。歯の外傷は主に前歯に発生するため、審美性の障害とさらなる再建の十分な接着が重要です。したがって、本稿の目的は、複合樹脂材料を検出および除去するための効率的かつ簡単なアプローチとしてのFIT法の適用を実証することです。

プロトコル

この研究で使用された歯は、地元の倫理委員会(EKNZ UBE-15 / 111)によって承認されたプロジェクトの一部でした。参加者は書面によるインフォームドコンセントを提供し、患者の機密性を保護するためにすべてのデータが匿名化されました。

1. FITを用いた歯牙色コンポジットレジン材料の検出

  1. 部屋を暗くします(自然光と人工光)。
  2. UV保護付きの透明または黄色がかった安全メガネを着用してください。
  3. 蛍光誘導光源を使用して、歯の物質と歯の色のコンポジットレジン修復物を照らします(図1)。
    注意: コンポジットレジン材料は、歯の物質よりも明るく見えます(図2)。波長398±5nmの任意のデバイスを蛍光誘導光源として使用できます。ヘッドランプシステムは、光スポットが口腔全体を照らし、同時に触覚検査を可能にするため、特に適しているようです。唾液や歯垢などの閉塞因子はFIT法を妨げません。したがって、以前の洗浄や歯の繰り返しの乾燥は必要ありません。

2. FITを用いたコンポジットレジン結合トラウマスプリントの除去

  1. 口腔内スキャン装置と実験評価のための適切なソフトウェアによる術前スキャン
    1. 口腔内スキャンデバイスを起動し、ソフトウェアを開きます( 資料表を参照)。
    2. [新しい患者の追加]を押して患者を登録し、姓、名、生年月日、および患者IDのギャップを埋めて、[ケースの追加]を押します。
    3. [表示]セクションの下にある[ジョースキャンインプレッション]を選択します。次に、[次へ]を押します。
    4. 部屋を暗くし(自然光と人工光)、操作フィールドを乾燥させて、スキャン手順を容易にします。
    5. 口腔内スキャナーを起動し、操作フィールドのデジタル表面スキャンを実行します(図3A)。
  2. コンポジットレジン材料の可視化
    1. 手順1.1〜1.3を繰り返します。
    2. 一般的な方法(ダイヤモンドバーと研磨装置を備えた高速コントラアングルハンドピースなど)を使用してコンポジットレジン材料を除去します(図4)。
      注意: 剥離用に設計された超硬バーを使用して、エナメル質の近くにあるコンポジットレジンの残りを取り除きます。
  3. 実験的な体積評価のための術後スキャン
    1. 手順 2.1.1-2.1.5 を繰り返します。
