本記事では、メンデルランダム化(MR)、トランスクリプトミクス解析、ネットワーク薬理学を組み合わせた統合的な手法を解説し、変形性関節症(OA)の治療対象を特定し優先順位付けします。このプログラムは、このマルチモーダルパイプラインを導入し、OAに対する薬物再利用や新規治療介入の開発を調査する研究者を導くことを目的としています。この方法論的枠組みは5段階の連続的段階で構成されています。まず、2サンプルMRを用いたMR解析パイプラインでOAに関連する推定因果血漿タンパク質を特定し、次にSteigerフィルタリングおよび現象全体の関連スキャンを行い、因果的方向性および潜在的なオフターゲット効果を評価します。第二に、RNAシークエンシングデータを統合して候補タンパク質標的を特定・精製するトランスクリプトミックシーケンシングワークフロー、第三に、MRとトランスクリプトムの結果を統合して候補タンパク質の優先順位付けを行う統合戦略;第四に、ネットワーク薬理学および分子ドッキング手法で、タンパク質間相互作用ネットワークの構築、機能富集解析、分子ドッキングによるリガンド-標的相互作用の探求;第五は意図された応用で、治療的関連性のある候補化合物や天然産物の優先順位付けに焦点を当てています。ワークフローを明確な分析段階に組織することで、このフレームワークは遺伝学的およびトランスクリプトムの発見から計算的薬物標的評価への移行を再現可能にし、具体的な実験結果を提示せずに提供します。この方法論は、公開されているデータセットと計算ツールを用いて、OA治療薬の体系的かつ仮説に基づく調査を促進します。

変形性関節症(OA)は軟骨の進行性劣化、軟骨下骨の変化、過度な骨の成長を特徴とします現在では、世界中で障害や経済的負担の大きな原因となっています。現在の治療法は主にステロイドまたは非ステロイド性抗炎症薬(NSAIDs)を用いて痛みや炎症を和らげています。しかし、NSAIDsは消化器系や心血管系に望ましくない副作用を引き起こすことが多い3.関節置換手術は代替手段を提供しますが、約3分の1の患者は顕著な痛みの緩和や機能改善を経験しません。OAの分子メカニズムの理解が深まるにつれ、いくつかの有望な治療標的が浮上し、より精密かつ個別化された治療が可能になっています5.ヒトタンパク質が生物学的プロセスにおいて重要な役割を果たすことから、これらはOA薬の開発の重要な候補となります。Nelsonらは、遺伝子支持標的の選択が臨床開発における薬剤の高い失敗率の解決に役立つ可能性があると示唆しています6

メンデルランダム化(MR)は、ゲノムワイド関連研究(GWAS)の単一塩基多型(SNP)を計測変数として用いて、曝露とアウトカムの因果関係を推定する遺伝学的手法です。観察研究と比較して、MRは受精時の遺伝子型のランダムな組み合わせを活用し、交絡因子を減らし因果関係を逆転させます感度解析は多遺伝子多面性を説明し、因果関係を近似する推論を提供します。これらの強みにより、MRは多発性硬化症や関節リウマチなどの疾患の治療標的を特定するためにますます活用されています。しかし、MRに基づく予測は依然として実験的な検証を必要とし、トランスクリプトミクス解析を通じて主要な疾患関連遺伝子の優先順位付けが可能です。

ネットワーク薬理学は、システム生物学と計算ツールを組み合わせた学際的アプローチであり、病気は単一遺伝子変異ではなく生物学的ネットワークの乱れによって生じることが多いと提案します。12。この手法は、多成分薬剤の解析と、薬物・疾患データベースをマイニングすることでシステムレベルでの治療標的の予測を可能にします13。このフレームワークの重要な要素である分子ドッキングは、薬剤と標的間の結合親和性をシミュレートし、薬剤の可薬性を評価するものです。これらの進展にもかかわらず、MRとネットワーク薬理学をOAに統合した研究はほとんどありません。

本記事では、MR、トランスクリプトミクス解析、ネットワーク薬理学を統合し、OAの有効な治療標的として血漿タンパク質を特定するプロトコルを提示します。目的は、原因タンパク質の特定だけでなく、計算解析を通じて治療用化合物候補の優先順位をつけることです。

南昌大学第一附属病院の生医学研究倫理委員会から倫理的承認とインフォームドコンセントを取得しました。倫理番号:(2025)CDYFYYLK(08-007)。

MR解析

データ検索
プラズマpQTLデータは、Zhengらによる研究(5つのGWASデータセット1516171819)およびFerkingstadらによる研究から取得されました。データの対象基準は以下の通りです:(i) ゲノム全体で有意な関連(p < 5 × 10⁻⁸);および(ii)OAの潜在的な治療標的としての血漿タンパク質。研究デザインは図1にまとめられています。まず、IEU OpenGWASのGWASデータとZheng14およびFerkingstad20の研究(補足表S1および補足表S2)の血漿pQTLデータを用いて候補治療標的を特定しました。その後、シュタイガーフィルタリングと表現型スキャンを用いて結果の堅牢性を検証しました。IEU OpenGWAS(https://gwas.mrcieu.ac.uk/)は、股関節または膝のOA(n = 417,596)、膝のOA(n = 403,124)、および股関節OA(n = 393,873)の要約統計を取得するために使用されました21

SNPフィルタリングコマンド

ゲノム全体有意(p < 5 × 10⁻⁸)を持つSNPは、MR解析前に凝集過程(r² < 0.001、F統計量>10、ウィンドウサイズ=10,000 kb)を受けました。

MR解析

潜在的な薬物標的を調査するために、血漿タンパク質を曝露として、OAを結果としてMR解析を行い、R(v4.3.1)の「TwoSampleMR」パッケージで実装されました。タンパク質に対して単一のpQTLしか利用可能な場合、Wald比が用いられました。それ以外の場合は、逆分散加重MR(MR-IVW)を適用し、その後異質性および多面性評価を行いました。ボンフェローニ補正は複数回の検査を考慮し、タンパク質の優先順位付けにはp < 5.63×10⁻⁵の閾値が適用されました。

シュタイガーフィルタリングと表現型スキャン

逆因果性を評価するために、私たちはシュタイガーフィルタリングを実施しました。「TRUE」とp < 0.05の結果は逆因果関係がないことを示しました。表現型スキャンはLDtrait (https://ldlink.nih.gov/?tab=ldtrait#home-tab)22 を用いてpQTLsと他の形質との関連を調べました。閾値はR² = 0.1と±500,000塩基対のウィンドウでした。多面性効果は、以下の条件を満たすpQTLsに割り当てられました:(i) ゲノム全体で有意な関連(p < 5 × 10⁻⁸)、および (ii) 既知のOAリスク因子との関連。

現象全体の関連研究

遺伝子多面性およびオフターゲット効果を考慮するために、アストラゼネカのPheWASポータル(https://azphewas.com/)を用いて現象全体連合研究(PheWAS)を実施しました。このポータルには、約45万人の英国バイオバンク参加者から15,500の二項表現型と1,500の連続表現型が含まれています。誤検知を最小限に抑えるために閾値がデフォルト値に設定されました。

タンパク質-タンパク質相互作用(PPI)ネットワーク

MRで特定された潜在的なタンパク質標的間の相互作用を可視化するため、GeneMANIA(https://genemania.org/)を用いてタンパク質間相互作用解析と結果の可視化を行いました

濃縮分析

生物学的関連性を調査するため、 https://www.bioinformatics.com.cn のバイオインフォマティクスツールを用いてデータ解析と可視化を行いました。

トランスクリプトミックワークフロー

製造元のガイドラインに従い、RNA抽出試薬キットを用いて総RNAを抽出しました。RNA品質は自動RNA品質評価システムを用いて評価されました。使用されたサンプルはRIN ≥7.0のみでした。品質はRNaseフリーアガロースゲル電気泳動(1.5%ゲル)によって確認されました。真核生物のmRNAはオリゴ(dT)ビーズを用いて濃縮され、原核生物mRNAはRNA脱離磁気キットを用いて濃縮されました。mRNAは断片化(200〜700 nt)され、RNAライブラリープレップキットを用いてcDNAに変換されました。cDNAライブラリーはエンドリペア、Aテール、アダプターへの接続、DNA精製磁気ビーズ(1.0×)による精製、PCR増幅が行われました。シーケンスは高スループットの次世代シーケンスプラットフォーム上で行われました。差異発現遺伝子はlog₂FC >1で定義され、p <0.05に調整されました。

ネットワーク薬理学

標的タンパク質の潜在的な薬剤を特定するために、BATMAN-TCM (http://bionet.ncpsb.org.cn/batman-tcm/index.php)25を用いました。既知および予測化合物の選定にはスコアカットオフ0.74(LR = 32.5)が用いられました。TCMSP(https://old.tcmsp-e.com/index.php)からハーブ成分を取り出し、OB>30%、DL>0.1826でろ過しました。

分子ドッキング

結合相互作用を評価するために分子ドッキングが用いられました。PDBからタンパク質構造を抽出しました(https://www.rcsb.org/)。UCSFキメラは配位子と溶媒を除去して構造の前処理に用いられました。AutoDock Toolsを使ってガスタイガー充電を計算し、ボックスの中心とサイズを定義しました。薬剤構造はPubChem(https://pubchem.ncbi.nlm.nih.gov/)から取得され、同様に前処理されました。ドッキングはAutoDock Vinaを使用して行われました。箱の寸法はターゲットによって異なっていました。結合親和性を計算し、その結果をUCSFキメラで可視化しました。

変形性関節症の原因タンパク質のプロテオームスクリーニング
