全身性エリテマトーデスにおけるマクロファージ:研究動向と新興フロンティアの文献計量分析

本研究は、Web of Science Core Collectionに基づく文献計量手法を用いて、2016年から2025年にかけて発表された全身性エリテマトーデス(SLE)におけるマクロファージの役割に関する1,197本の論文を体系的に分析しました。この分析では、出版傾向、共同ネットワーク、研究テーマ、知識基盤を検証し、現在の研究環境、中核研究力、そしてこの分野のフロンティアの進化を明らかにしました。論文数は2022年に154件でピークを迎え、年間引用数は2016年の184件から2025年には9,030件に増加し、活発で影響力のある研究を示しています。中国は論文数で1位(379本)となり、アメリカは総引用数(18,838)と国際的な共同研究の中心性でトップとなりました。上海交通大学とカロリンスカ研究所が最も生産的な機関で、それぞれ23本の論文を発表しました。免疫学のフロンティア(インパクトファクター=5.9;ジャーナル引用報告第1クォルタイルは最も生産的なジャーナルでした。キーワードタイムラインのクラスタリングでは、好中球細胞外トラップ、樹状細胞、トール様受容体などの自然免疫の主要成分や、T細胞、B細胞、形質細胞状樹状細胞を含む適応免疫を含む13の主要な研究テーマが特定されました。これらのテーマには、マクロファージ分極、NOD様受容体ファミリーであるピリンドメイン含有3(NLRP3)インフラマソーム、アポトーシス細胞クリアランスなどの中核的なメカニズムや、ループス腎炎、マクロファージ活性化症候群、関節リウマチなどの臨床的ホットスポットも含まれていました。研究ホットスポットの進化は、初期のアポトーシス細胞および免疫複合体のクリアランスという基本的なメカニズムから、サイトカイン、自然免疫、適応免疫の精緻な調節へと進化し、最終的にはシグナル伝達経路標的療法、臨床管理、多施設検証の研究へと進み、「経路」と「多施設診断」が現在の研究フロンティアとして際立っています。

概要

全身性エリテマトーデス(SLE)は、自己抗体の産生、免疫複合体の沈着、多臓器炎症を特徴とする慢性自己免疫疾患です近年、診断手法や治療戦略が大幅に進歩したにもかかわらず、SLEは腎臓、血液系、中枢神経系に深刻な損傷を与え続けています3,4,5。既存の治療法、グルココルチコイド、免疫抑制剤、生物学的製剤は一般的に治療効果が限定的であり、副作用の度合いは異なると関連しています。キメラ抗原受容体T(CAR-T)細胞療法などの細胞工学的手法は、自己反応性B細胞の深い枯渇を通じてSLEにおいて持続的な薬物不寛解を達成する可能性を示しています。マクロファージは自然免疫系の重要なメンバーとして、免疫複合体のクリアランス、アポトーシス細胞のエフェロサイトーシス、サイトカイン分泌において重要な役割を果たし、免疫恒常性を維持しています11,12。蓄積される証拠は、SLEの発症機序に複数の形態のマクロファージ機能障害が関与していることを示しています13,14,15。マクロファージの分極が炎症促進型M1型と抗炎症性M2型の差異を一貫して観察されており、ループス腎炎ではM1マクロファージが組織損傷を促進し、M2マクロファージは組織修復および線維化再構築に寄与しています16,17。好中球細胞外トラップ(NET)の過剰形成やインターフェロンシグナル伝達経路の異常活性化は、マクロファージ媒介の炎症反応をさらに増幅します(18,19,20)。その結果、マクロファージ機能障害はSLEの発症機序における中枢ノードとしてだけでなく、有望な治療標的としても浮上しています。

過去10年間で、SLEにおけるマクロファージの役割に焦点を当てた論文が急速に増加し、エフェロサイトーシス、NETosis、マクロファージ分極、代謝再プログラミング、シグナル伝達経路の標的調節など多様な研究方向に及びます。.しかし、増え続ける文献は断片的な研究テーマ、中核研究力の不明確な特定、フロンティアの進化の曖昧なマッピングといった課題ももたらしており、現在の研究状況の体系的な概観の緊急性を強調しています。Bibliometricsは数学的および統計的手法を用いて知識キャリアの分布特性や進化パターンを定量的に分析し、出版傾向、協働ネットワーク、研究ホットスポット、フロンティアダイナミクスに関する客観的なデータを提供します 27,28,29,30,31.文献計量学的アプローチはSLE免疫療法、バイオマーカー発見、関連分野の知識マッピングに成功裏に応用されていますが、SLEのマクロファージに焦点を当てた文献計量マッピングは依然として限界があります32,33,34。本研究は、2016年から2025年までのWeb of Scienceコアコレクションデータベースから関連文献を取得し、CiteSpace、VOSviewer、Scimago Graphicaを用いて多次元的な視覚分析を実施します。論文発表、国・地域および機関の協力、著者とジャーナルのネットワーク、キーワードの共生およびタイムラインクラスタリング、さらに引用バースト検出を検証することで、本研究はSLEにおけるマクロファージ分野における現在の研究状況、中核研究力、テーマ的進化、新たなフロンティアを体系的に明らかにし、将来の研究のための客観的な知識マップと方向性の参照を提供することを目指しています。

プロトコル

本研究では倫理的承認やインフォームド・コンセントは不要であり、すべてのデータはWeb of Scienceコアコレクションデータベース内の公開書誌記録から得られました。この研究には人間の参加者、動物、臨床サンプルは含まれていません。

データ取得および検索戦略

データの一貫性と再現性を確保するため、Web of Scienceコアコレクション(WOSCC)データベースから2026年4月27日に取得されました。検索クエリは以下の構造を持つブール演算子を用いていました:TS=("全身性ループス・エリテマトーデス"または"全身性エリテマトーデス"または"Lupus Erythematosus Disseminatus")およびTS=("マクロファージ*")。文書の種類は記事およびレビュー記事に限定されていました。出版期間は2016年1月1日から2025年12月31日まででした。合計1,197件の有効な論文が後続の文献計量分析のために保持されました。

解析ツール

複数のソフトウェアパッケージを統合した分析パイプラインが使用されました。CiteSpace(研究資源識別子[RRID]: SCR_020932)はキーワードタイムラインのクラスタリングおよび引用バースト分析に使用されました。35。VOSviewer(RRID: SCR_016598)は、国・地域、機関、著者の共著ネットワークの構築・可視化、キーワード共現マップの生成に使用されました。36。Scimago GraphicaはVOSviewer37からエクスポートされたネットワークファイルから国や地域のコラボレーションネットワーク可視化を生成するために使用されました。基本的な統計計算と図の準備はWPS ExcelとGraphPad Prism(RRID: SCR_002798)を用いて行われました。全体的な書誌計量のワークフローは 図1に示されています。

CiteSpaceおよびVOSviewerにおけるパラメータ設定

CiteSpaceでは、2016年から2025年までの期間を1年ごとに設定していました。g-インデックス(k = 10)がノード選択の閾値として選ばれました。Pathfinderの剪定アルゴリズムは2回適用されました。最初は各個別のタイムスライスネットワークを簡素化するため、もう一度はすべてのスライスを統合して視覚的複雑さを軽減するために適用しました。VOSviewerでは、ネットワークレイアウト最適化のためにLinLog/モジュール性アルゴリズムが選ばれました。ノードサイズは、特定の分析に応じて文書数や引用頻度で重み付けされました。Scimago Graphicaを用いた国・地域共同マッピングでは、ノードの直径は論文の量を反映し、リンクの太さは国間の協力強度を表していました。

結果

年間出版物の生産量

2016年から2025年の間に、SLEにおけるマクロファージに関する論文は合計1,197本発表されました。年間発行件数は変動し、2016年には112部から始まり、2022年には154部でピークを迎え、その後2025年にはわずかに減少して133部となりました。同期間、年間引用数は全体で184件から9,030件に増加し、わずかな変動があり、この分野における学術的影響力の段階的な増加を示しています(図2)。

国・地域貢献と国際協力ネットワーク

本書誌計量学的研究は、過去10年間(2016–2025年)におけるSLEマクロファージの研究状況を体系的に明らかにしています。Web of Scienceコアコレクションの1,197件の論文を分析することで、世界的な出版傾向、協働ネットワーク、コア研究力、テーマの進化、知識ベースの動態を特定し、この急速に進化する分野における将来の研究のための客観的なロードマップを提供しました。
