1. 術前プロトコル
<ol><li>診療所で脊椎病理を有する患者を評価し、脊椎手術の適格性を決定する。神経学的評価を行い、CTまたはMRIスキャンを取得して脊椎病変を同定する。</li>
	<li>シュワンノーマ、上衣腫、髄膜腫、星状細胞腫などの硬膜内病理を有する患者 を含む。または腹側胸部ヘルニア椎間板、腹側骨折断片、または腹側圧迫を伴う脊髄骨腫瘍などの腹側圧迫硬膜外病理を有する患者。 注:病理は、CTまたはMRIによる脊髄画像によって決定される。除外基準には、手術に耐えられない患者、または予後が極めて悪い患者が含まれる。</li>
</ol>2. 手術の準備
<ol><li>手術前の真夜中以降に患者が口で何かを消費することを許さないでください。 注:患者は全身麻酔下に置かれ、麻酔科医によって挿管される。</li>
	<li>脊椎手術に対する外科医の好みに応じて背中を露出させた患者を配置します。</li>
	<li>手術領域をポビドンヨードで消毒するには、その領域をこすります。</li>
</ol>3. 手術
注:プロトコルのこのセクションは、評判の良い脊椎手術技術の教科書19から参照できる一般的な脊椎手術技術に従います。
<ol><li>適切な椎骨レベルの上に背骨の長さに沿ってメスで切開を行い、骨に達するまでまっすぐな切開を続けます。 注:切開部の大きさは、病理の大きさに依存する。例えば、腫瘍が2つの椎体レベルにまたがる場合、少なくとも2つの椎体レベルを露出させる必要がある。骨が露出すると、ポータブルX線装置でX線を照射して、正しい椎骨を検証することができます。</li>
	<li>電気外科的焼灼による骨膜下郭清を行い、球根骨プロセスとして可視化される棘突起を露出させる。刃先を腹側に回し、層流を横切って掃引します。</li>
	<li>Leksellボーンプライヤーと高速ドリルを組み合わせて骨の薄層と棘状プロセスを除去し、その下の靭帯フラバムを露出させます。</li>

</ol>4. 術中超音波
<ol><li>移動式超音波装置と直径20mmのトランスデューサプローブを使用してください。 メモ: プローブの周波数範囲は 10 ~ 4.4 MHz である必要があります。プローブの直径と周波数範囲が類似した同等のデバイスであれば十分です。</li>
	<li>骨の除去および硬膜曝露の後、超音波トランスデューサプローブを水没させることができるように、手術野を十分な生理食塩水で満たしてください。 注:一般に、100〜500mLの生理食塩水が必要です。生理食塩水は音響結合を可能にします。</li>
	<li>超音波装置の電源を入れ、超音波プローブを生理食塩水浴内に関心のあるレベルに配置して、画像の取得を開始します。 注:プローブを硬膜または脊髄に直接触れる必要はありません。画像は超音波スクリーン上でリアルタイムで取得され、外科医によって直ちに解釈することができる。画面上の画像は、 フリーズ ボタンを押すことでいつでもキャプチャでき、保存ボタンを押すことで 保存 できます。</li>
	<li>超音波プローブを脊柱管の方向に沿って配置することにより、縦面のリアルタイム画像を取得し、MRIからの矢状画像と同様に脊髄および病変部を視覚化する。</li>
	<li>超音波プローブを脊柱管に垂直に配置して横平面のリアルタイム画像を取得し、MRIからの軸方向画像のように脊髄と病変部を可視化します。</li>
	<li>リアルタイム画像を取得して、直接可視化できない病変の位置確認、術前CTやMRI画像との相関、手術器具の配置の誘導、病理の分解能の確認を行います。 注:必要に応じて、約0.5 cm x 0.5 cmの滅菌圧縮スポンジの小片を、手術野に配置する過エコー手術マーカーとして使用し、手術位置と画像位置を相関させるのに役立ちます。これは、手術中に病変を見つけるのに役立ち、腫瘍の縁を特定するのにも役立ちます。</li>
</ol>5. 術後フォローアップ
