すべてのロボットトロカーの間に少なくとも9センチメートルの距離を確保してください。内視鏡と右側のポートの間に、より珊瑚的なレベルで追加の腹腔鏡トロカールを配置します。セットアップについては、図 2 を参照してください。
アームカードを1つにして、傾斜角度をマイナス30度、ドッキング角度を40度に調整します。アームカード2を傾斜角度0度、ドッキング角度110度にセットアップしますアームカードの表示を常に制御して、最適なセットアップを確保します。外科医の左手にはバイポーラグラスパーを、外科医の右手にはモノポーラ湾曲シャーを、4本目の腕にはグラスパーを装備します。
手順のロボット部分の開始時。4本目の腕で胃をつかんで持ち上げ、バイポーラグラスパーとモノポーラシャーと血管シーリング装置を組み合わせて胃結腸靭帯を切開します。凝固後の腫瘍の3センチメートル下から3センチメートル上まで、器具のコントロールを使用して胃エピカラー靭帯を切断します。
腫瘍は、正常な胃組織と比較して、その形状と表面的な外観によって識別できます。腹腔鏡ステープラーを使用して腫瘍を切除します。小さな一歩を踏み出すだけで、組織をしっかりと引っ張ることで、できるだけ多くの胃組織を節約します。
その結果、胃の体と眼底の横方向の広い切除が行われます。シングルノットを使用してステープルラインを補強します。吸収性縫合糸を使用し、腹腔鏡下で余った縫合糸を切断し、縫合糸に固定されていた針を抜く。
内視鏡の位置でミニ開腹術による回収バッグを使用して腫瘍を切除します。ドッキング時間は13分でしたが、コンソールの時間は115分かかりました。術中の合併症やロボットの誤動作はなく、失血もほとんどありませんでした。
術後3時間回復室で経過観察し、4日後に退院した。手術前に腫瘍に中枢性壊死を認めた内視鏡所見を図3に示します。組織学的検査は腫瘍の切除後に実施されました。
神経鞘腫が明らかになり、完全に切除されました。追加の治療や再入院は必要ありませんでした。3ヵ月後の経過観察では再発の兆候は認められなかった。
図5から術前および術後の上腹部のCTスキャンでは、腫瘍の切除が成功したことが強調されています。表2は、異なるロボットシステムを使用して実施された胃部分切除の類似症例を比較したものである。手術期間、腫瘍の大きさ、入院期間についても同様の値が提示された。
結論として、胃広域切除の場合、新しいモジュール式ロボットプラットフォームは安全で実現可能でした。さらに、開発された方法は、楽しいアプリケーションやヘテロプラスティなどの他の外科的目的にも適しており、一般化可能性と再現性の両方を保証します。
