腹腔鏡下左外側切片切除術:肝靭帯と臍裂静脈による指導

Laparoscopic Left Lateral Sectionectomy: Guided by the Ligamentum Teres Hepatis and the Umbilical Fissure Vein

代表的な症例では、手術時間は120分で、推定失血量は50mLで、術後合併症はありませんでした。術後の入院期間は7日間でした。 表 1 は、術中および術後のデータをまとめたものです。術後5日目に実施されたコンピューター断層撮影(CT)では、肝臓部分に血液や体液が蓄積している証拠は見られませんでした(図3)。手術後、無事に退院しました。標本の組織学的検査により、肝内胆管結石が明らかになり、炎症性細胞の浸潤と肝門領域の小さな胆管の増殖が認められました(図4)。患者は、感染することなく、満足のいく術後切開治癒を示しました(図5)。
