The Immediate Partial Removal of Cumulus-Oocyte Complexes: A Refined Approach for Rapid Observation of In Vitro Fertilization

短期間の共インキュベーション手順を受けているグループでは、レスキューを受けた患者は除外されました。短期授精を受けた患者47人のうち、患者の年齢と回収された卵子の数に有意差は認められませんでした(表1)。OCCCの即時部分的な除去により、卵母細胞の周囲に残っていたOCCCが緩み(図3)、第2の極性体の検出が容易になりました。OCCC群の部分的切除では、平均処置時間は対照群と比較して有意に短かった(4.42±1.68分対9.29±5.04分、 P &lt;0.001)が、切断率、最適胚率、最適胚盤胞率に有意差は認められなかった。
従来の 体外 受精(IVF)を受けた68人の患者の別のコホートでは、受精チェックが授精後18〜20時間で実施され、患者の年齢と2つのグループ間で回収された卵子の数に類似性が認められました(表2)。卵子を囲む顆粒膜細胞の構造の緩みの増加は、採卵中にOCCCの即時部分除去を受けた患者で観察されました(図4)。OCCC群の部分的な除去では、平均運転時間がわずかに短いことが認められた(4.93 ± 0.80分対6.54 ± 7.77分、 P = 0.207)、統計的に有意な差は見られなかった。場合によっては、密集した接着により、観察手順が40分を超える長期にわたることにつながる可能性がありますが、一晩保管した後のほとんどのケースでOCCCの接着が一般的に緩んでいることは、2つのグループ間の操作時間に統計的なばらつきがないことを説明できる可能性があります。前のグループと同様に、切断率、最適な胚率、および最適な胚盤胞率に大きな不一致は確認されませんでした。
全体として、代表的な結果は、OCCCの即時部分的除去が、卵割率、最適な胚率、最適な胚盤胞率などの主要なIVF結果を損なうことなく、短期授精手順の手術時間を大幅に短縮することを示しています。この手法は、卵子の周りに残っているOCCCを緩めることにより、第2極性体の検出を容易にし、手術時間の短縮に貢献している可能性があります。従来のIVF手順の文脈では、OCCCの即時部分除去により手術時間が短縮される傾向が観察されましたが、その差は統計的に有意ではありませんでした。これらの知見は、OCCCの即時部分的切除は、特に短期授精の状況において、手続きの効率を向上させることができる一方で、従来の体外受精のタイムラインへの影響はそれほど顕著ではない可能性があることを示唆しています。
