The Role of Anatomical Dissection in Defining Colic and Small Bowel Artery Lymphovascular Bundles in the D3 Volume of Small and Large Bowel Mesentery

スイスのジュネーブ大学医学部解剖学ユニットの遺体ドナープログラムからの合計 3 つの Jores 防腐処理された遺体 (59 歳、70 歳、84 歳の 3 人の女性全員、A、B、C とマーク) が研究に含まれました。3つのケースすべてで、微細な解剖は、D3ボリュームのリンパ管とリンパ節、および上腸間膜血管軸およびそれらの枝と豊かさとの関係の包括的で総観的なビューを取得しました。疝痛枝に関しては、いずれの症例にも右疝痛動脈は存在しませんでした。空腸動脈の数はA = 4、B = 5、C = 4でした。解剖領域の下限には、最大3本の回腸動脈とそれに隣接するリンパ管が含まれていました。リンパ管網には、腸動脈に隣接するリンパ管束、吻合枝、束の間を流れる独立した枝、そして最後に、上腸間膜動脈の左縁に沿って流れる一定のコレクターリンパ管が含まれ、大腸(右)と小腸(左)からドレナージを受けます。平均リンパクリアランス、つまり、リンパ管束内の特定の動脈とその隣接するリンパ管の間の距離を 表 1 に示します。データは、Statistica 64-bit v. 14.0.0.15 (TIBCO Software Inc. 2020) を使用して統計分析を受けました。小さなサンプルは、2 変数のマンホイットニー U 検定を使用して修飾されました。空腸動脈クリアランスに関して、3 つのグループ間に統計的差はありませんでした (p &gt; 0.05)。3 つの症例の回腸クリアランスを比較した場合、同じ結果 (p &gt; 0.05) が得られました。一方、すべての空腸クリアランス (まとめて) を疝痛 (回結腸および中疝痛) クリアランスと比較した場合、サンプル サイズによりスチューデントの t 検定の使用が可能になりました。疝痛クリアランスは小腸のクリアランスよりも有意に大きかった(t値14.35、df = 26、p&lt;0.05)。3例のうち2例で、空腸静脈が上腸間膜動脈と空腸動脈を前方に横切る様子が観察されました。頭蓋方向の解剖では、上空腸動脈と下膵頭十二指腸動脈の総幹の症例は見られず、上腸間膜動脈リンパ鞘と中疝痛動脈左枝に続くリンパ管が明確に分離されました。
