食道空腸吻合術による胃全摘術後の吻合部漏出の治療のための内視鏡的真空療法

Endoscopic Vacuum Therapy for the Treatment of Anastomotic Leakage after Total Gastrectomy with Esophagojejunostomy

この代表的なケースでは、患者は EVTDD 置換術を 3 回受けました。EVTDD 配置の平均処置時間は 37 分で、施設での総入院期間は 21 日でした。EVT 処置中または手術後に合併症は観察されませんでした。
最初のEVT処置中に、内視鏡検査により、食道空腸吻合部で1cm×1.5cmの漏出が明らかになり、黄白色の化膿性コーティングに囲まれ、縫合糸と血管クリップが目に見えることが明らかになりました(図5)。EVTDDは手術中に適切に配置され、十分なドレナージが確保されました(図6)。術後、EVT ドレナージは効果的に機能し、患者の両側腹部ドレナージ チューブから少量の化膿性液体が排出され、障害は受けられませんでした。2回目のEVT処置中に、内視鏡検査の結果、漏出が縮小し、端の周囲に軽度の粘膜出血が観察されたことが明らかになりました(図7)。患者の腹部ドレナージチューブからの毎日のドレナージ量は、0 m...
