Respiratory syncytial virus disease is a common respiratory illness in children. It is typically caused by the respiratory syncytial virus, or RSV.
The virus spreads between humans primarily through respiratory droplets, entering the host via the nasal mucosa.
After entry, RSV targets the apical surface of ciliated epithelial cells in the respiratory tract.
The virus envelope contains the G protein, which binds to cellular receptors, and the F protein, which initiates fusion of viral and host membranes.
After fusion, the viral nucleocapsid enters the host cell cytoplasm.
RSV first transcribes its RNA into mRNA, which directs viral protein production for capsid assembly and genome replication. The virus then uses the host’s cellular machinery to complete replication inside the cell.
Next, the new virions bud off to infect neighboring cells.
The host immune system responds by releasing inflammatory cytokines, which contribute to damage of the respiratory lining.
This causes cellular debris and mucus accumulation in the respiratory tract, which obstructs the airways and impairs normal breathing.
