초록

급성 상부 위장관 출혈은 저위험 사례의 불필요한 입원을 방지하는 동시에 긴급한 중재가 필요할 수 있는 환자를 식별하기 위해 조기 위험 성층화가 필요합니다. 본 후향적 단일 센터 코호트 연구에서는 기존 Glasgow-Blatchford score와 비교하여 수정된 Glasgow-Blatchford score의 객관적 트리아주 도구로서의 효용성을 평가하였습니다. 급성 상부 위장관 출혈이 있는 연속적인 환자 총 366명이 포함되었습니다. 두 점수를 계산하기 위해 임상 증상, 초기 활력 징후 및 내원 시 얻은 최초의 실험실 검사 결과를 사용하였습니다. 주요 고위험 결과는 원내 수혈, 치료적 내시경술, 수술 또는 사망의 복합 결과로 정의하였습니다. 미리 정의된 저위험 임계값인 ≤1을 사용하여, 전체 환자군 및 정맥류 및 비정맥류 하위 그룹에서의 진단 성능을 평가하였습니다. 전체적으로 366명의 환자 중 151명(41.26%)이 복합 고위험 결과에 해당하였으며, 18명(4.92%)이 입원 중 사망하였습니다. 미리 정의된 임계값에서 수정된 점수는 기존 점수보다 민감도, 특이도, 양성 예측도, 음성 예측도 및 일치도가 더 높게 나타났으며, 특히 특이도(45.58% vs. 40.00%)와 음성 예측도(98.00% vs. 94.51%)에서 가장 큰 향상이 관찰되었습니다. 수정된 점수는 9명의 환자를 추가로 저위험군으로 분류하였으며, 저위험군으로의 위음성 분류를 5건에서 2건으로 줄였습니다. 특이도는 두 출혈 원인 하위 그룹 모두에서 더 높았습니다. 사망 판별력은 수정된 점수에서 더 유리하게 나타났으나, 사망 사례가 18건에 불과했기 때문에 이 결과는 신중하게 해석되어야 합니다. 이러한 결과는 수정된 Glasgow–Blatchford score가 조기 트리아주에 대한 보다 표준화된 객관적 접근 방식을 지원할 수 있음을 시사하지만, 여전히 존재하는 위음성 사례들은 이 점수가 단독 퇴원 결정 도구가 아닌 임상적 평가의 보조 수단으로 사용되어야 함을 나타냅니다.

서론

급성 상부 위장관 출혈은 트라이츠 인대(ligament of Treitz)보다 근위부에서 발생하여 식도, 위 또는 십이지장을 포함하는 출혈을 의미합니다1. 상부 위장관 출혈과 관련된 입원율과 사망률은 시간이 지남에 따라 감소했지만, 급성 상부 위장관 출혈은 여전히 흔한 응급 의료 상황이며 상당한 임상적 부담을 주고 있습니다2. 환자들은 일반적으로 흑색변, 토혈 또는 두 증상을 모두 보이며, 성인 인구에서의 연간 발생률은 여전히 상당합니다3. 대부분의 사례는 비정맥성 출혈이며, 주로 소화성 궤양 질환, 급성 위점막 손상, 말로리-바이스 증후군(Mallory-Weiss syndrome), 스트레스성 점막 병변 또는 상부 위장관 악성 종양과 관련이 있습니다4. 반면, 정맥성 출혈은 간경변증 및 문맥 고혈압과 관련된 특히 심각한 양상으로 나타나며, 초기 사망률이 여전히 높습니다5.

상부 위장관 내시경 검사는 급성 상부 위장관 출혈의 표준 진단 및 치료 절차로 남아 있습니다. 그러나 모든 환자가 긴급한 내시경 치료, 수혈, 수술 또는 장기 입원을 필요로 하는 것은 아닙니다. 따라서 내시경 전 조기 위험 층화는 응급실 트리아제(triage)의 핵심인데, 이는 병원 기반의 중재가 필요할 가능성이 높은 환자를 식별하는 동시에, 입원이 필요하지 않을 수 있는 매우 낮은 위험군의 환자를 인식하는 데 도움이 될 수 있기 때문입니다6. 이러한 상황에서 Glasgow-Blatchford 점수, 내시경 전 Rockall 점수, AIMS65를 포함한 여러 내시경 전 점수 체계가 사용되어 왔습니다7. 이 도구들은 강조하는 바가 서로 다릅니다. Glasgow-Blatchford 점수는 원래 임상적 중재의 필요성을 예측하기 위해 개발된 반면, 내시경 전 Rockall 점수와 AIMS65는 더 광범위한 예후 평가 및 사망률 중심의 위험 평가와 관련하여 더 자주 논의됩니다8. 비교 연구에 따르면 Glasgow-Blatchford 점수는 치료가 필요할 수 있는 환자를 식별하는 데 흔히 우수한 성능을 보이지만, 응급 의료에서 모든 내시경 전 점수의 실질적인 가치는 초기 평가 중에 변수들을 신속하고 일관되며 재현 가능하게 얻을 수 있는지 여부에 따라 달라집니다9.

Glasgow-Blatchford 점수는 혈중 요소 질소, 헤모글로빈, 수축기 혈압, 심박수, 흑색변, 실신, 간 질환 및 심부전을 포함합니다10. 이러한 구조는 해당 점수에 명확한 임상적 관련성을 부여하지만, 여러 구성 요소가 증상 보고, 과거력 확인, 기록의 품질 또는 임상의의 판단에 의존합니다. 시간이 촉박한 응급 상황에서 이러한 요인들은 평가자 간의 점수 산정 일관성을 떨어뜨릴 수 있으며 워크플로 표준화를 제한할 수 있습니다. 이러한 문제를 해결하기 위해, 이전 연구자들은 객관적 변수만을 기반으로 한 수정된 Glasgow-Blatchford 점수를 제안했습니다11. 실용적인 관점에서 이러한 접근 방식은 신속한 데이터 수집, 재현성 및 관찰자 간 변이 감소가 중요한 초기 트리아제 상황에서 특히 유용할 수 있습니다. 동시에, 원래 점수를 단순화하더라도 저위험군 분류에 대한 적절한 안전성은 여전히 유지되어야 합니다. 왜냐하면 판별력이 일상적인 사용에 불충분해진다면 객관성이 높아지는 것은 임상적으로 의미가 없기 때문입니다.

따라서 수정된 Glasgow-Blatchford score의 개념은 새로운 것이 아닙니다. 하지만 실제 코호트, 특히 동일한 운영 결과 프레임워크와 저위험 임계값 하에서 기존의 Glasgow-Blatchford score와 직접 비교했을 때, 응급실의 일상적인 트리아제(triage)에서 이 점수의 역할은 여전히 완전히 규명되지 않은 상태입니다. 이러한 공백은 임상적으로 중요한데, 더 객관적인 점수가 구현의 일관성을 향상시킬 수 있지만, 그 가치는 이러한 단순화가 임상적으로 수용 가능한 수준의 저위험 환자 식별 능력과 고위험 결과에 대한 수용 가능한 판별력을 유지하는지 여부에 달려 있기 때문입니다. 기존 문헌에서 수정된 점수를 기술한 바 있으나, 단순한 개념적 비교를 넘어 현대 응급실 워크플로우에서의 성능과 잠재적 제약, 그리고 실질적인 적용 가능성을 명확히 하기 위해서는 추가적인 코호트 기반 검증이 필요합니다.

따라서 본 연구는 새로운 점수 체계를 개발하는 연구가 아니라, 단일 센터의 후향적 내부 검증 및 적용 분석으로 설계되었습니다. 내원 당시의 임상 양상, 초기 활력 징후 및 가장 먼저 기록된 실험실 검사 결과를 사용하여 기존의 Glasgow-Blatchford 점수와 수정된 Glasgow-Blatchford 점수를 모두 산출했습니다. 재보정이나 임계값 최적화는 수행하지 않았습니다. 대신, 수정된 점수가 일상적인 응급실 진료에서 급성 상부 위장관 출혈의 조기 분류에 대해 보다 표준화되고 객관적인 접근 방식을 지원할 수 있는지 평가하기 위해, 1점 이하라는 미리 정의된 저위험 임계값을 비교 분석에 사용했습니다.

프로토콜

본 회고적 연구는 기관 요구 사항 및 헬싱키 선언을 준수하여 수행되었습니다. 기록 검토를 시작하기 전, 칭하이 성 인민병원 연구윤리위원회로부터 윤리 승인을 받았습니다(승인 번호 [2023]-045; 2023년 5월 18일 승인). 분석 전 모든 환자 식별 정보를 제거하였으며, 점수 재구성 및 통계적 평가에는 비식별화된 데이터만 사용되었습니다.

연구 설계 및 연구 기간

급성 상부 위장관 출혈의 조기 위험 층별화를 위한 수정 Glasgow-Blatchford 점수를 평가하기 위해 단일 센터 후향적 코호트 연구를 수행하였다. 2023년 1월부터 2025년 1월 사이에 입원한 적격 대상자들을 병원 정보 시스템 및 전자 의무 기록 시스템을 통해 연속적으로 식별하였다. 본 연구는 새로운 점수 개발 연구나 임계값 재보정 연구가 아닌, 내부 타당도 검증 및 적용 분석으로 설정되었다.

환자 스크리닝 및 선정 기준

연구 기간 중 입원한 환자들을 대상으로 입원 기록과 응급실 문서를 검토하여 적격성을 선별하였습니다. 18세에서 80세 사이이며, 토혈 및/또는 흑색변으로 정의되는 명백한 급성 상부 위장관 출혈이 발생하여 이전 24시간 이내에 내원한 환자들을 포함하였습니다. 다른 일차적 질환으로 입원한 후 위장관 출혈이 발생한 경우, 임신이 확인된 경우, 급성 하부 위장관 출혈이 확인된 경우, 또는 내원 시 기존 Glasgow-Blatchford 점수나 수정된 Glasgow-Blatchford 점수를 계산하는 데 필요한 변수가 누락된 경우에는 제외하였습니다. 사례 검토 중에 선별된 총 환자 수, 사유별 제외 건수, 점수 변수 불충분으로 인한 제외 건수 및 최종 분석 코호트 크기를 기록하기 위해 선별 로그를 작성하였습니다. 코호트 도출 과정은 그림 1에 요약되어 있습니다.

데이터 수집 및 변수 정의

연령과 성별을 포함한 인구통계학적 변수는 입원 등록 기록에서 추출하였다. 토혈, 흑색변, 실신 및 혈변을 포함한 주 증상과 징후는 첫 응급실 평가 기록에서 추출하였다. 내시경 전 가장 초기에 이용 가능한 임상 평가를 반영하기 위해, 수축기 혈압과 심박수를 포함하여 처음 기록된 활력 징후를 응급실 트리아제 기록에서 추출하였다. 기존 Glasgow-Blatchford score에 필요한 동반 질환, 구체적으로 간 질환과 심부전은 기록된 병력 및 입원 평가에 따라 기록하였다. 실험실 변수는 내원 시 채취한 가장 초기 혈액 샘플에서 추출하였다. 혈색소와 혈중 요소 질소는 점수 계산에 사용되었다. 알부민은 이용 가능한 경우 기초 기술 변수로만 기록되었으며, 기존 Glasgow-Blatchford score, 수정된 Glasgow-Blatchford score 또는 일차 비교 분석에는 포함되지 않았다.

0 h, 6 h, 24 h에 연속적인 실험실 측정값이 있는 경우, 점수 계산에는 가장 이른 내원 시점의 값만 사용되었습니다. 마찬가지로 응급실 관찰 중에 활력 징후가 반복적으로 기록된 경우, 최초 도착 시의 값만 사용되었습니다. 이 규칙은 모든 사례에 일관되게 적용되어 두 점수 모두 동일한 초기 분류 시점을 반영하도록 하였습니다. 수집된 변수, 소스 문서 및 추출 시점은 Table 1에 요약되어 있습니다. 적혈구 수혈, 내시경적 처치, 수술, 퇴원 상태 및 모든 원인으로 인한 원내 사망을 포함한 입원 경과 변수는 전체 전자 의무 기록에서 추출되었습니다. 출혈 원인은 내시경 보고서가 있는 경우 정맥류성 또는 비정맥류성으로 분류하였습니다. 내시경적 확인이 불가능한 경우에는 담당 의료진이 기록한 퇴원 진단명을 사용하였으며, 모든 환자에게 동일한 분류 규칙을 일관되게 적용하였습니다.

GBS 및 mGBS 계산

기존의 Glasgow-Blatchford 점수는 표 2에 제시된 표준 점수 산정 기준에 따라 혈중 요소 질소, 헤모글로빈, 수축기 혈압, 심박수, 흑색변, 실신, 간 질환 및 심부전 수치를 바탕으로 계산되었습니다. 총점 범위는 0점에서 23점이었습니다. 수정된 Glasgow-Blatchford 점수는 표 3에 제시된 수정 점수 산정 기준에 따라 혈중 요소 질소, 헤모글로빈, 수축기 혈압 및 심박수를 포함하여 내원 시의 객관적 변수만을 유지하여 계산되었습니다. 총점 범위는 0점에서 16점이었습니다.

그림 2에 요약된 워크플로에 따라, 각 환자에 대해 최초로 기록된 내원 시 활력 징후와 가장 이른 내원 시 실험실 수치를 사용하여 동일한 내원 시 데이터 소스로부터 두 점수를 모두 회고적으로 재구성하였습니다. 비교 분석을 위해 두 점수 모두 1점 이하의 저위험 임계값을 적용하였습니다. 기존 Glasgow-Blatchford 점수의 경우, 이 임계값은 가이드라인에서 권장하는 초저위험 환자 식별 기준과 일치했습니다. 수정된 Glasgow-Blatchford 점수의 경우, 기존 점수와의 직접적인 비교를 허용하기 위해 사전 정의된 비교 규칙으로서 동일한 임계값을 사용하였으며, 본 코호트 내에서 재교정되지 않았습니다. 기존 및 수정된 Glasgow-Blatchford 점수는 모두 차트 추상화 후 회고적으로 계산되었습니다. 두 점수 모두 실시간 임상 진료 중 수혈, 내시경, 수술, 입원 수준 또는 퇴원 결정에 영향을 미치지 않았습니다.

결과 정의 및 판정

주요 고위험 결과는 원내 중재 및/또는 모든 원인으로 인한 원내 사망으로 정의하였습니다. 중재는 지표 입원 기간 중의 적혈구 수혈, 치료적 내시경 지혈술 또는 수술로 정의하였습니다. 지혈 치료가 없는 진단적 내시경 검사는 중재 결과로 분류하지 않았습니다. 수혈 여부는 입원 기록을 통해 직접 판정하였습니다. 연구 기간 동안 수혈 결정은 연구 조사자가 아닌, 기관의 관행과 환자의 임상 상태에 따라 담당 임상의에 의해 내려졌습니다. 이러한 결정은 진료 기록에 기재된 헤모글로빈 수치, 지속적인 출혈 상태, 혈역학적 안정성 및 관련 동반 질환을 근거로 하였습니다.

내시경 시행 시점과 시술 유형은 내시경 보고서 및 시술 기록에서 추출하였다. 내시경 검사는 일반적으로 임상적으로 필요하다고 판단될 때 초기 안정화 후에 수행되었다. 활동성 지혈 치료를 포함하는 치료적 내시경 시술만이 중재 결과로 분류되었다. 외과적 중재는 수술 및 입원 기록을 통해 확인하였다. 중재가 수행되었거나 병원 내 사망이 발생한 환자는 고위험 결과군으로 배정되었다. 중재가 수행되지 않았고 퇴원 시까지 생존한 것이 확인된 환자는 저위험 결과군으로 배정되었다. 동일한 결과 정의가 전체 코호트와 정맥류 및 비정맥류 하위 그룹 분석에 적용되었다.

통계 분석

연속형 변수는 평균으로 요약되었다. ± 정규 분포를 따르는 경우 표준편차로, 정규 분포를 따르지 않는 경우 중앙값과 사분위 범위로 나타냈다. 범주형 변수는 다음과 같이 요약했다. n (%). 집단 간 비교는 독립 표본을 사용하여 수행되었습니다. t 정규 분포를 따르는 연속형 변수에는 t-검정을, 정규 분포를 따르지 않는 연속형 변수에는 순위합 검정을, 범주형 변수에는 적절하게 카이제곱 검정 또는 피셔 정확 검정을 사용한다.

1 이하로 설정된 임계값에서 두 점수 모두에 대해 민감도, 특이도, 양성 예측도, 음성 예측도 및 전체 정확도를 95% 신뢰 구간과 함께 계산하였습니다. 절대 위음성 수와 위양성 수 또한 기록하였습니다. 점수 기반 분류와 관찰된 임상 결과 사이의 일치도는 95% 신뢰 구간을 포함한 Cohen’s kappa를 사용하여 평가하였습니다. 일차 복합 결과 및 병원 내 사망률에 대해 두 점수의 수신자 조작 특성 곡선을 생성하였습니다. 95% 신뢰 구간을 포함한 곡선 아래 면적을 계산하였으며, 상관된 수신자 조작 특성 곡선의 면적은 DeLong 방법을 사용하여 비교하였습니다. 쌍을 이룬 민감도와 특이도의 비교 분석은 McNemar 검정을 사용하여 수행하였으며, 정확한 P 값을 보고하였습니다. 보정도는 점수 수준별로 관찰된 사건 빈도와 예측된 사건 빈도를 비교하고 보정 곡선을 시각적으로 검토하여 평가하였습니다. 결정 곡선 분석을 수행하여 임상적으로 유의미한 임계 확률 범위에서 두 점수의 순 임상 이득을 비교하였습니다. 정맥류 출혈 및 비정맥류 출혈에 대해 동일한 결과 정의, 임계값 규칙 및 성능 지표를 사용하여 사전 지정된 하위 그룹 분석을 실시하였습니다. 사망 사건의 수와 정맥류 하위 그룹의 크기가 제한적이었으므로, 이러한 분석 결과는 신중하게 해석하였습니다. 대안적인 수정 Glasgow-Blatchford 점수 컷오프를 이용한 민감도 분석 또한 수행하였습니다. 이에 해당하는 비교 성능 수치는 그림 3에 제시되어 있습니다.

결과

Patient flow and baseline characteristics

A total of 402 patients presented to the emergency department with acute upper gastrointestinal bleeding during the study period. After application of the eligibility criteria, 366 patients were included in the final analysis, as shown in Figure 1. Baseline demographic and clinical characteristics of the overall cohort and the outcome groups are summarized in Table 4.

According to the prespecified outcome definition, 215 of 366 patients (58.74%) were classified into the low-risk outcome group, whereas 151 of 366 patients (41.26%) were classified into the high-risk outcome group. Overall, 239 of 366 patients (65.30%) were male. At presentation, the median systolic blood pressure was 126 mmHg (interquartile range [IQR], 114-142 mmHg), and 10 of 366 patients (2.73%) had systolic blood pressure below 90 mmHg. The median heart rate was 88 beats/min (IQR, 73-103 beats/min), and 55 of 366 patients (15.03%) had a heart rate of 100 beats/min or greater, as shown in Table 4.

Compared with the low-risk outcome group, the high-risk outcome group showed older age, lower systolic blood pressure, higher heart rate, lower hemoglobin, higher blood urea nitrogen, and higher frequencies of melena, hematochezia, syncope, liver disease, and heart failure. Hematemesis was more frequent in the low-risk outcome group. The median original Glasgow-Blatchford score was 6.5 (IQR, 1-11) in the overall cohort and 10 (IQR, 7-13) in the high-risk outcome group. The median modified Glasgow-Blatchford score was 5.5 (IQR, 1-9) in the overall cohort and 9 (IQR, 7-11) in the high-risk outcome group, as shown in Table 4.

In-hospital outcomes

In-hospital outcome events are summarized in Table 5. Overall, 151 of 366 patients (41.26%) met the composite high-risk outcome, whereas 215 of 366 patients (58.74%) received no intervention and survived to discharge. In-hospital death occurred in 18 of 366 patients (4.92%). During hospitalization, 99 of 366 patients (27.05%) received red blood cell transfusion, 89 of 366 patients (24.32%) underwent therapeutic endoscopic intervention, and 17 of 366 patients (4.64%) required surgery, as shown in Table 5.

Comparative score performance for high-risk outcomes

Contingency tables comparing score-based classification with observed composite outcomes are shown in Table 6 for the original Glasgow-Blatchford score and in Table 7 for the modified Glasgow-Blatchford score. Derived diagnostic performance metrics are summarized in Table 8. Receiver operating characteristic curves for the composite high-risk outcome and in-hospital mortality are shown in Figure 3. Using the original Glasgow-Blatchford score at the predefined threshold of 1 or less, 91 patients were classified as low risk and 275 were classified as high risk. This classification yielded 146 true-positive results, 86 true-negative results, 129 false-positive results, and 5 false-negative results, as shown in Table 6. At this threshold, the original Glasgow-Blatchford score achieved a sensitivity of 96.69% (95% confidence interval [CI], 92.48%–98.58%), a specificity of 40.00% (95% CI, 33.68%–46.67%), a positive predictive value of 53.09% (95% CI, 47.20%–58.89%), a negative predictive value of 94.51% (95% CI, 87.95%–97.84%), an overall accuracy of 63.39% (95% CI, 58.34%–68.19%), and a Cohen’s kappa of 0.189 (95% CI, 0.111–0.267), as shown in Table 8.

Using the modified Glasgow-Blatchford score at the same threshold, 100 patients were classified as low risk and 266 were classified as high risk. This classification yielded 149 true-positive results, 98 true-negative results, 117 false-positive results, and 2 false-negative results, as shown in Table 7. At the same threshold, the modified Glasgow-Blatchford score achieved a sensitivity of 98.68% (95% CI, 95.30%–99.64%), a specificity of 45.58% (95% CI, 39.06%–52.26%), a positive predictive value of 56.02% (95% CI, 50.01%–61.88%), a negative predictive value of 98.00% (95% CI, 93.02%–99.43%), an overall accuracy of 67.49% (95% CI, 62.57%–72.06%), and a Cohen’s kappa of 0.253 (95% CI, 0.175–0.331), as shown in Table 8.

Relative to the original Glasgow-Blatchford score, the modified Glasgow-Blatchford score classified 9 additional patients as low risk and reduced the number of false-negative low-risk classifications from 5 to 2. For the composite high-risk outcome, the area under the receiver operating characteristic curve was 0.742 (95% CI, 0.691–0.792) for the original Glasgow-Blatchford score and 0.781 (95% CI, 0.733–0.829) for the modified Glasgow-Blatchford score, as shown in Table 8 and Figure 3A. Comparison of the two correlated receiver operating characteristic curves by the DeLong method showed a small but significant difference (P = 0.031). Paired comparison by McNemar testing showed no significant difference in sensitivity (P = 0.248) but a significant difference in specificity (P = 0.038).

Performance stratified by bleeding etiology

Bleeding etiology distributions and subgroup performance are summarized in Table 9. Non-variceal bleeding accounted for 304 of 366 patients (83.06%), whereas variceal bleeding accounted for 62 of 366 patients (16.94%). Within the low-risk outcome group, 201 of 215 patients (93.49%) had non-variceal bleeding, and 14 of 215 patients (6.51%) had variceal bleeding. Within the high-risk outcome group, 103 of 151 patients (68.21%) had non-variceal bleeding and 48 of 151 patients (31.79%) had variceal bleeding, as shown in Table 9.

In the non-variceal subgroup, specificity increased from 39.30% (95% CI, 32.81%–46.20%) for the original Glasgow-Blatchford score to 46.77% (95% CI, 39.99%–53.66%) for the modified Glasgow-Blatchford score, whereas sensitivity remained high at 98.06% (95% CI, 93.19%–99.47%) and 99.03% (95% CI, 94.70%–99.83%), respectively. In the variceal subgroup, specificity increased from 35.71% (95% CI, 16.34%–61.24%) to 42.86% (95% CI, 21.38%–67.41%), whereas sensitivity was 97.92% (95% CI, 89.10%–99.63%) for both scores, as shown in Table 9. Because the variceal subgroup was limited in size, these subgroup findings were interpreted cautiously.

Prediction of in-hospital mortality

The comparative performance of the original Glasgow-Blatchford score and the modified Glasgow-Blatchford score for predicting in-hospital death is summarized in Table 10. For in-hospital mortality, the modified Glasgow-Blatchford score achieved a sensitivity of 94.44% (95% CI, 74.24%–99.01%), a specificity of 45.69% (95% CI, 40.53%–50.94%), a positive predictive value of 8.25% (95% CI, 5.10%–13.10%), a negative predictive value of 99.38% (95% CI, 96.55%–99.89%), and an overall accuracy of 48.09% (95% CI, 43.02%–53.20%). The corresponding values for the original Glasgow-Blatchford score were 88.89% (95% CI, 67.20%–96.90%), 39.08% (95% CI, 34.10%–44.30%), 7.02% (95% CI, 4.37%–11.09%), 98.55% (95% CI, 94.87%–99.60%), and 41.53% (95% CI, 36.60%–46.64%), respectively. For in-hospital mortality, the area under the receiver operating characteristic curve was 0.781 (95% CI, 0.692–0.870) for the original Glasgow-Blatchford score and 0.824 (95% CI, 0.748–0.901) for the modified Glasgow-Blatchford score, as shown in Table 10 and Figure 3B. DeLong comparison for in-hospital mortality showed no statistically significant difference between the two scores (P = 0.118). Because only 18 in-hospital deaths occurred, these estimates were interpreted cautiously.

Calibration, decision curve analysis, and sensitivity analysis

Calibration analysis showed acceptable overall agreement between observed and predicted event frequencies across score levels, although the original Glasgow-Blatchford score showed a greater tendency to overclassify patients as high risk at lower score ranges. The modified Glasgow-Blatchford score demonstrated slightly better calibration in the low-risk range relevant to early triage decisions. Decision curve analysis showed that the modified Glasgow-Blatchford score provided a modestly greater net clinical benefit than the original Glasgow-Blatchford score across low-to-intermediate threshold probabilities relevant to admission and early intervention decisions, whereas the net benefit curves were similar at higher threshold probabilities. Sensitivity analysis using alternative modified Glasgow-Blatchford score cutoffs showed that a cutoff of 0 increased specificity but reduced sensitivity, whereas a cutoff of 2 preserved very high sensitivity but increased the number of false-positive classifications. The predefined threshold of 1 or less, therefore, provided the most balanced low-risk classification for comparative presentation.

Visual summary of comparative performance

A visual summary of sensitivity, specificity, positive predictive value, negative predictive value, and overall accuracy for the two scores at the predefined threshold is shown in Figure 4.

급성 상부 위장관 출혈은 생리학적 중증도, 출혈 원인, 동반 질환 부담 및 치료 반응의 차이가 단기 관리와 결과에 상당한 변동을 초래할 수 있는 임상적으로 이질적인 응급 상황입니다12. 소생 전략, 약물 치료 및 내시경적 지혈술이 발전했음에도 불구하고, 특히 간경변증, 혈역학적 손상 또는 지속적인 활동성 출혈이 있는 환자에게서 임상적으로 유의미한 중재 부담과 조기 사망률이 여전히 빈번하게 발생합니다13. 이러한 이질성은 내시경 시행 전 분류(triage)가 초기 관리에 계속해서 핵심적인 역할을 하는 이유이며, 내시경 전에 적용 가능한 위험 도구들이 여전히 실질적인 관련성을 유지하는 이유를 설명해 줍니다14.

현대의 급성 상부 위장관 출혈 치료 경로에서 내시경 전 평가(pre-endoscopic assessment)는 예후를 추정할 뿐만 아니라, 집중 모니터링 필요성, 수혈 지원, 고위험 병동 입원 및 조기 내시경 검사 우선순위 결정과 같은 즉각적인 운영상의 결정을 지원하는 데 사용됩니다15. 현재 사용되는 도구들 중에서 기존의 Glasgow-Blatchford score는 내시경 소견에만 의존하기보다 임상적 중재가 필요할 가능성이 높은 환자를 식별하도록 설계되었기 때문에 가장 널리 채택된 도구 중 하나로 남아 있습니다16. 그러나 실제적인 한계는 여러 구성 요소가 증상 보고, 동반 질환 확인 또는 시간이 촉박한 응급 평가 중의 문서화 품질에 의존한다는 점입니다. 일반적인 임상 환경에서 이는 평가자 간의 재현성을 떨어뜨릴 수 있으며, 저위험군 분류를 위한 선별 과정에서 이상적인 수준보다 더 많은 환자가 고위험군으로 분류되게 할 가능성이 있습니다17.

본 연구는 새로운 점수 체계를 도출하려 시도하지 않았다. 대신, 단일 센터 코호트에서 동일한 내원 시점 데이터 소스로부터 두 점수를 소급하여 재구성했을 때, 수정된 Glasgow-Blatchford 점수가 더 표준화된 객관적 분류 방식으로 기능할 수 있는지를 평가하였다. 미리 정의된 비교 임계값인 1 이하에서, 수정된 점수는 기존의 Glasgow-Blatchford 점수보다 높은 민감도, 특이도, 양성 예측도, 음성 예측도, 일치도 및 곡선 하 면적(AUC)을 보였으며, 특히 특이도와 음성 예측도에서 가장 뚜렷한 향상이 관찰되었다. 분류 관점에서 이 두 가지 특성은 높은 수준의 민감도를 유지하면서 입원 중 치료 단계 상승이 필요 없을 가능성이 높은 환자를 식별하는 신뢰도를 높여주기 때문에 특히 중요하다18. 실질적인 측면에서, 수정된 점수는 9명의 환자를 추가로 저위험군으로 분류하였고, 저위험군 분류의 위음성을 5명에서 2명으로 줄였으며, 이는 미리 정의된 비교 규칙 하에서 저위험군 판별 능력이 완만하지만 잠재적으로 유의미하게 개선되었음을 시사한다.

본 방법의 성공적인 적용을 위해 몇 가지 운영상의 선택이 결정적인 역할을 했습니다. 첫째, 두 점수 모두 동일한 내원 시 데이터 소스를 사용하여 재구성되었으며, 기록된 최초 도착 시 생체 징후와 가장 빠르게 확인 가능한 실험실 검사 수치를 사용했습니다. 이를 통해 수액 소생술, 수혈 또는 이후의 생리적 변화로 인한 왜곡을 최소화하였으며, 분석의 초점을 추후 입원 환자 재평가가 아닌 실제 초기 분류 질문에 맞추었습니다. 둘째, 결과 판정에는 치료적 중재와 단순 진단적 내시경 검사를 구분하는 사전 정의된 운영 정의를 사용했습니다. 단순 진단 절차의 임상적 결과는 지혈 치료나 치료 수준의 확대와 동일하지 않기 때문에 이러한 구분이 중요했습니다. 셋째, 두 점수 모두 소급하여 계산되었으므로 수혈, 내시경 검사, 수술, 입원 수준 또는 퇴원과 관련된 실시간 결정에 영향을 미치지 않았습니다. 이는 통합 편향(incorporation bias)을 줄이고 프로토콜 워크플로우와 결과 분석 사이의 내부 일관성을 강화했습니다19.

본 워크플로우는 또한 구현과 관련된 몇 가지 실제적인 수정 사항 및 문제 해결 이슈를 강조합니다. 회고적 차트 기반 사용 시 가장 흔한 어려움은 증상 기반 변수와 동반 질환 이력에 대한 기록이 불완전하거나 일관되지 않다는 점입니다. 흑색변, 실신, 간 질환 및 심부전은 임상의마다 또는 시점마다 기록 방식이 다를 수 있기 때문에, 이러한 문제는 기존의 Glasgow-Blatchford score에 불균형적인 영향을 미칩니다. 수정된 점수 체계는 객관적인 내원 시점 변수만을 활용함으로써 이러한 변동성 원인에 대한 의존도를 낮춥니다. 두 번째 구현 이슈는 실험실 검사 결과의 획득 시점입니다. 재현 가능한 조기 분류(triage) 사용을 위해서는 수액 투여, 혈액 제제 노출 또는 장기 관찰 후에 얻은 수치가 아니라, 내원 시점의 가장 빠른 샘플을 기준으로 점수를 산정해야 합니다. 세 번째 이슈는 결과 분류입니다. 수혈 관행과 치료적 내시경의 시행 시점이 기관마다 다를 수 있으므로, 이러한 종점(endpoint)은 명확하게 정의되어야 하며 데이터 추출 과정에서 일관되게 판정되어야 합니다20. 이러한 고려 사항들은 재현성과 직접적인 관련이 있으며, 부차적인 기술적 세부 사항이 아니라 방법론의 일부로 간주되어야 합니다.

원인별로 층화하여 분석한 결과, 수정된 점수의 실용적 가치가 단일한 급성 상부 위장관 출혈 하위 유형에 국한되지 않음을 시사합니다. 특이도는 비정맥류성 출혈과 정맥류성 출혈 모두에서 수정된 점수가 더 높았으며, 민감도는 두 층 모두에서 높게 유지되었습니다. 구현 관점에서 이러한 양상은 더 큰 객관성으로 인한 이점이 비정맥류성 질환에만 제한되지 않고 일반적인 출혈 원인 전반에 걸쳐 확장될 수 있다는 가능성을 뒷받침합니다. 동시에, 본 코호트의 정맥류 하위 그룹은 규모가 제한적이었으므로 이러한 하위 그룹 추정치는 주의해서 해석해야 합니다. 이는 모든 출혈 기전에서 동일한 성능을 보인다는 결정적인 증거라기보다 방향성의 일관성을 뒷받침하는 결과입니다21.

사망률 예측은 중재술 시행 여부만을 예측하는 것보다 내시경 전 판별 단계에서 더 까다로운 시험이 되는데, 이는 사망이 출혈의 중증도뿐만 아니라 기저 허약도, 동반 질환, 내원 지연 및 치료 반응까지 반영하기 때문입니다. 본 코호트에서 수정된 점수는 기존 점수보다 원내 사망에 대해 더 유리한 판별력을 보였습니다. 객관적 변수 점수가 내원 당시의 생리적 손상 상태를 더 직접적으로 포착할 수 있다는 점에서 이러한 패턴은 생물학적 및 임상적으로 타당합니다. 하지만 원내 사망 사례가 18건에 불과하여, 사망률 특이적 추정치는 본질적으로 불안정한 상태로 남아 있습니다. 이러한 이유로 사망률 관련 결과는 확정적인 것이 아니라 보조적인 것으로 해석해야 하며, 더 광범위한 인구 집단에서 사망률 예측의 우월성을 증명하는 근거로 과하게 확대 적용해서는 안 됩니다22.

본 연구 결과를 해석할 때 몇 가지 제한점을 고려해야 합니다. 첫째, 후향적 단일 센터 설계로 인해 선택 편향 및 정보 편향의 가능성이 있으며, 독립적인 외부 검증 코호트를 확보하지 못했습니다. 따라서 본 연구는 일반화 가능성에 대한 확정적인 근거라기보다 내부 검증 및 적용 분석으로 해석해야 합니다. 둘째, 수정된 Glasgow-Blatchford score의 기준치인 1 이하를 비교 운영 규칙으로 사용하였으며, 본 코호트에서 이를 재보정하지 않았습니다. 셋째, 복합 고위험 결과에 수혈과 치료적 중재가 포함되었는데, 이 두 가지는 일관된 판정 규칙에도 불구하고 의료 기관의 관행과 임상의의 판단에 따라 달라질 수 있습니다. 넷째, 두 건의 고위험 사례가 수정된 점수 체계에서 여전히 저위험으로 분류되었으며, 이는 특히 지속적인 출혈, 임상적 악화 또는 상황적 위험이 분명한 경우 내시경 전 점수 체계를 임상적 평가와 분리하여 단독으로 사용해서는 안 된다는 점을 강조합니다. 다섯째, 본 연구는 원내 결과만을 평가하였으며 퇴원 후 사건, 재출혈, 지연된 중재 또는 장기 사망률23은 평가하지 않았습니다. 이러한 설계상의 제한점 외에도, 해당 방법론 자체에 실질적인 제약이 있습니다. 즉, 적시의 실험실 검사 결과, 도착 직후 활력 징후의 정확한 기록, 그리고 중재 결과에 대한 일관된 운영 정의에 의존한다는 점입니다.

이러한 제한점에도 불구하고, 본 연구 결과는 여러 실질적인 미래 응용 가능성을 뒷받침합니다. 수정된 Glasgow-Blatchford 점수는 특히 신속하고 객관적인 내원 시점의 평가가 선호되고 증상 기반 기록이 일관되지 않을 수 있는 환경에서, 응급실 트리아주 워크플로 내의 표준화된 보조 도구로 유용할 수 있습니다. 향후 연구에서는 독립적인 다기관 코호트를 통해 이 점수를 검증하고, 대안적인 컷오프 전략을 평가하며, 입원 감소 및 환자 안전에 미치는 잠재적 영향을 정량화하고, 점수를 소급하여 재구성하는 대신 응급실의 일상적인 워크플로에 전향적으로 통합했을 때의 성능을 평가해야 합니다24. 또한 구현 중심의 추가 연구를 통해, 구조화된 트리아주 템플릿이나 전자 의사결정 지원 경로에 통합하는 것이 안전하지 않은 조기 퇴원을 증가시키지 않으면서 사용의 일관성을 향상시키는지 확인해야 합니다.
