전신성 홍반성 루푸스의 대식세포: 연구 동향과 신흥 개척지에 대한 서지계량학 분석

이 연구는 Web of Science Core Collection을 기반으로 한 서지계량 방법을 사용하여 2016년부터 2025년까지 발표된 전신성 홍반성 루푸스(SLE)에서 대식세포의 역할에 관한 1,197편의 논문을 체계적으로 분석하였습니다. 분석은 출판 동향, 협력 네트워크, 연구 주제, 지식 기반을 검토하여 현재 연구 환경, 핵심 연구 세력, 그리고 이 분야의 최전선적 진화를 명확히 했습니다. 논문 수는 2022년에 154편으로 정점을 찍었으며, 연간 인용 건수는 2016년 184회에서 2025년 9,030회로 증가하여 활발하고 영향력 있는 연구를 보여주었습니다. 중국은 출판 수(379편)에서 1위를 차지했으며, 미국은 총 인용(18,838회)과 국제 협력 중심성에서 선두를 차지했습니다. 상하이 교통대학교와 카롤린스카 연구소가 각각 23편의 논문을 발표하며 가장 생산적인 기관이었다. 면역학의 최전선(임팩트 팩터 = 5.9; 저널 인용 보고서 쿼타일 1)는 가장 생산적인 저널이었습니다. 키워드 타임라인 클러스터링은 호중구 세포외 함정, 수지상 세포, 톨 유사 수용체 등 선천 면역의 핵심 구성 요소와 T 세포, B 세포, 형상세포 수지상 세포 등 13개의 주요 연구 주제를 확인했습니다. 이 주제에는 대식세포 분극, NOD 유사 수용체 계열인 피린 도메인 3(NLRP3) 인접종, 세포자멸사 세포 제거와 같은 핵심 기전뿐만 아니라 루푸스 신염, 대식세포 활성화 증후군, 류마티스 관절염과 같은 임상 핫스팟도 포함되었습니다. 연구 핫스팟의 진화는 세포 세포 및 면역 복합체 제거라는 초기 기본 메커니즘에서 사이토카인, 선천면역, 적응 면역의 정교한 조절로, 그리고 최종적으로 신호전달 경로 표적 치료, 임상 관리, 다기관 검증 연구로 발전한 과정을 반영하며, '경로'와 '다기관 진단'이 현재 연구 최전선으로 두드러지고 있습니다.

서론

전신성 홍반성 루푸스(SLE)는 자가항체 생성, 면역 복합체 침착, 다기관 염증을 특징으로 하는 만성 자가면역 질환입니다 1,2. 최근 수십 년간 진단 접근법과 치료 전략의 상당한 발전에도 불구하고, SLE는 신장, 혈액계, 중추신경계에 심각한 손상을 계속 일으키고 있습니다 3,4,5. 글루코코르티코이드, 면역억제제, 생물학적 제제 등 기존 치료법들은 일반적으로 치료 효능이 제한적이며 다양한 정도로 부작용과 연관되어 있습니다 6,7,8. 키메라 항원 수용체 T(CAR-T) 세포 치료와 같은 세포공학 전략은 자가반응성 B 세포의 깊은 고갈을 통해 SLE에서 지속 가능한 약물 없는 관해를 달성할 잠재력을 보여주었습니다 9,10,11. 선천 면역계의 핵심 구성원으로서, 대식세포는 면역 복합체 제거, 세포자멸사 세포의 발화세포 형성, 사이토카인 분비에 중요한 역할을 하여 면역 항상성을 유지합니다11,12. 누적된 증거는 SLE 발병 기전과 여러 형태의 대식세포 기능 장애가 관련이 있음을 시사합니다 13,14,15. 루푸스 신염에서는 M1 대식세포가 조직 손상을 촉진하는 반면, M2 대식세포는 조직 손상을 촉진하는 반면, M2 대식세포는 조직 복구와 섬유화 재형성에 기여하는 루푸스 신염에서 일관되게 관찰되었습니다16,17. 호중구 세포외 트랩(NET)의 과도한 형성과 인터페론 신호 전달 경로의 비정상적인 활성화는 대식세포 매개 염증 반응을 더욱 증폭시킵니다 18,19,20. 그 결과, 대식세포 기능 장애는 SLE 발병 기전의 중심 결절일 뿐만 아니라 유망한치료 표적으로 부상했습니다.

지난 10년간 대식세포가 SLE에서 역할을 다룬 논문이 급격히 증가했으며, 이는 발세포작용, NETosis, 대식세포 분극, 대사 재프로그래밍, 신호 전달 경로의 표적 조절 등 다양한 연구 방향을 아우르는 연구입니다22,23,24,25,26. 그러나 급증하는 문헌은 단편화된 연구 주제, 핵심 연구 세력의 불분명한 식별, 최전선 진화의 모호한 지도 등 도전 과제도 함께 제시하며, 현재 연구 환경에 대한 체계적인 개요가 시급히 필요함을 부각시킨다. Bibliometrics는 수학적·통계적 방법을 활용하여 지식 운반자의 분포 특성과 진화 패턴을 정량적으로 분석하며, 출판 동향, 협력 네트워크, 연구 핫스팟, 그리고 최전선의 역학에 대한 객관적인 데이터를 제공합니다 27,28,29,30,31. 서지계량학 접근법은 SLE 면역치료, 바이오마커 발견 및 관련 분야에서 지식 매핑에 성공적으로 적용되었으나, SLE의 대식세포에 초점을 맞춘 서지계량 매핑은 여전히 제한적입니다 32,33,34. 이 연구는 2016년부터 2025년까지 Web of Science의 핵심 컬렉션 데이터베이스에서 관련 문헌을 수집하고, CiteSpace, VOSviewer, Scimago Graphica를 활용해 다차원 시각적 분석을 수행합니다. 출판 결과물, 국가/지역 및 기관 협력, 저자 및 학술지 네트워크, 키워드 공현 및 타임라인 군집화, 인용 폭발 탐지를 분석함으로써, 본 연구는 SLE 대식세포 분야의 현재 연구 상태, 핵심 연구 세력, 주제 진화, 그리고 신흥 개척지를 체계적으로 밝혀내어 향후 연구를 위한 객관적인 지식 지도와 방향성 참조를 제공하는 것을 목표로 합니다.

데이터 수집 및 검색 전략

데이터는 데이터 일관성과 재현성을 보장하기 위해 Web of Science Core Collection(WOSCC) 데이터베이스에서 단 한 날짜(2026년 4월 27일)에 검색되었습니다. 검색 쿼리는 다음과 같은 구조의 불리언 연산자를 사용했습니다: TS=("루푸스 홍반, 전신" 또는 "전신성 루푸스 에리테마토스" 또는 "루푸스 홍반 전포성") 그리고 TS=("대식세포*"). 문서 유형은 논문과 리뷰 논문으로 제한되었습니다. 출판 기간은 2016년 1월 1일부터 2025년 12월 31일까지였습니다. 총 1,197편의 유효 논문이 이후 서지 분석을 위해 보관되었습니다.

분석 도구

여러 소프트웨어 패키지를 통합한 분석 파이프라인이 사용되었습니다. CiteSpace(연구 자원 식별자 [RRID]: SCR_020932)는 키워드 타임라인 클러스터링과 인용 버스트 분석에 사용되었습니다.35. VOSviewer(RRID: SCR_016598)는 국가/지역, 기관, 저자에 대한 공동 저자 네트워크를 구축하고 시각화하는 데 사용되었으며, 키워드 공동 출현 지도36을 생성하는 데 사용되었습니다. Scimago Graphica는 VOSviewer37에서 내보내는 네트워크 파일을 기반으로 국가/지역 협업 네트워크 시각화를 생성하는 데 사용되었습니다. 기본 통계 계산과 도표 작성은 WPS Excel과 GraphPad Prism(RRID: SCR_002798)을 사용해 수행되었습니다. 전체 서지 측정 워크플로우는 그림 1에 나와 있습니다.

CiteSpace 및 VOSviewer에서의 매개변수 구성

CiteSpace에서는 2016년부터 2025년까지 1년 간격으로 시간 단편이 설정되었습니다. g-인덱스(k = 10)가 노드 선택 임계값으로 선택되었습니다. Pathfinder 가지치기 알고리즘은 두 번 적용되었습니다: 첫 번째는 각 개별 시간 슬라이스 네트워크를 단순화하기 위해서였고, 두 번째는 시각적 복잡성을 줄이기 위해 모든 슬라이스를 병합한 후에 적용되었습니다. VOSviewer에서는 네트워크 레이아웃 최적화를 위해 LinLog/모듈러리티 알고리즘이 선택되었습니다. 노드 크기는 특정 분석에 따라 문서 수나 인용 빈도에 따라 가중치가 부여되었습니다. Scimago Graphica를 이용한 국가/지역 협업 매핑의 경우, 노드 직경은 출판물 양을 반영하고, 링크 두께는 국가 간 협업 강도를 나타냈습니다.

연간 출판물
