Acetylcholine Accumulation

Acetylcholine accumulation is the excessive buildup of the neurotransmitter acetylcholine at synapses and neuromuscular junctions, a pharmacological condition that disrupts normal cholinergic signaling. It most often occurs when acetylcholinesterase, the enzyme that rapidly hydrolyzes acetylcholine, is inhibited by drugs or toxicants, allowing continuous stimulation of muscarinic and nicotinic receptors. This mechanism underlies the effects of therapeutic cholinesterase inhibitors and the toxicity of organophosphate and carbamate compounds. Studying acetylcholine accumulation helps explain cholinergic symptoms, evaluate drug actions, and guide the development of treatments that restore balanced neurotransmission.

Intracoronary acetylcholine testing has been established for the assessment of epicardial coronary spasm more than 30 years ago. Recently, the focus has shifted towards the microcirculation and it has been shown that microvascular spasm can be detected using ACH-testing. This article describes the ACH-test and its implementation in daily routine.

This protocol presents the acetylcholine rechallenge after nitroglycerine as an add-on procedure to spasm provocation testing. The purpose of this technique is to unmask co-existing microvascular spasm in patients with epicardial spasm and to assess the protective efficacy of nitroglycerine on a per-patient level to guide medical therapy.

This video demonstrates an assay to measure the accumulation of a fluorescent antibiotic probe in bacterial cells. Exposure to an efflux pump inhibitor disrupts the transmembrane proton gradient in bacterial cells, upon which a fluorophore-conjugated antibiotic is added. Inhibition of efflux pumps due to the disrupted proton gradient causes accumulation of the antibiotic inside the bacteria. Bacterial cells are lysed, and the fluorescence intensity is measured to confirm antibiotic accumulation.
