</ol>5. 통계 분석

전 세계적으로 중요한 건강 문제인 패혈증은 감염에 대한 숙주 반응 조절 장애로 인해 생명을 위협하는 장기 기능 장애로 정의됩니다. 글로벌 질병 부담 연구(Global Burden of Diseases Study)에 따르면 2017년 전 세계적으로 4,890만 건의 패혈증 사례와 1,100만 명의 패혈증 관련 사망자가 발생한 것으로 추정되었으며, 이는 전세계 사망자의 거의 20%를 차지합니다1. 사망을 유발하는 혈류 감염(BSI)의 약 2/3는 그람 음성 세균 병원체에 의한 것이다2. 그람음성병원체(GN)의 주요 사망 원인은 대장균(Escherichia coli), 폐렴균( Klebsiella pneumoniae), 녹농균(Pseudomonas aeruginosa), 아시네토박터 바우마니(Acinetobacter baumannii)이며, 이들은 33종의 세균성 병원체 중 발병 사례의 약 40%를 차지한다2.
혈액 배양은 BSI 진단을 위한 황금 표준으로 남아 있으며, 항균제 감수성 검사(AST) 결과와 함께 신속한 미생물 식별이 관리의 핵심입니다. 패혈증에서 적절한 항균제를 투여하는 데 시간이 걸릴 때마다 사망 확률이 9% 증가하는 것으로 추정된다3. AST 결과와 함께 미생물학적으로 양성 혈액 배양 보고서의 처리 시간(TAT)은 자동화된 시스템에서도 리소스가 제한된 환경에서 사용 가능한 미생물 도구를 사용할 경우 약 48-72시간입니다. 이러한 수준 이하의 TAT로 인해 광범위한 항생제가 경험적으로 사용되어 항생제 내성(AMR) 문제가 급증하고 있습니다. 패혈증에 대한 미생물 배양 기법에 대한 TAT를 줄여야 할 필요성을 인식한 EUCAST와 CLSI는 양성 플래그가 있는 혈액 배양 병(+aBC)에서 직접 AST를 수행하는 방향으로 나아가고 있습니다(+aBC)4,5.

Direct Microbial Identification using An Automated Microbial Identification System to Facilitate the EUCAST RAST Method Without Mass Spectrometry

그람 음성(GN) 패혈증은 자원이 제한된 환경에서 3-4일 내에 결과를 제공하는 기존의 미생물 배양 기술에 의존하는 의료 응급 상황입니다. 이러한 처리 시간(TAT)의 지연을 인식하여 EUCAST와 CLSI는 양성 플래그가 지정된 자동 혈액 배양 병(+aBC)에서 직접 AST 결과를 결정하기 위한 프로토콜을 개발했습니다. EUCAST rapid AST(RAST) 프로토콜은 2018년에 처음 도입되었으며, GN 패혈증의 4가지 일반적인 원인 병원체, 즉 대장균, 폐렴균, 녹농균, 아시네토박터 바우마니 복합체에 대한 구역 지름 중단점을 보고할 수 있습니다. 그러나 일상적인 워크플로우에서 이 방법을 구현한 임상 실험실은 쉽게 사용할 수 없는 질량 분석 기반 미생물 식별에 의존하므로 자원이 제한된 환경에서 구현할 수 없습니다. 이를 피하기 위해 상용 자동 미생물 식별 및 항균제 감수성 검사 시스템(aMIAST)을 사용하여 직접 접종 프로토콜(DIP)을 평가하여 aBC 양성 플래그 후 8시간 이내에 미생물을 조기에 식별할 수 있도록 했습니다. 2023년 1월부터 10월까지 이 프로토콜을 평가하여 긍정적으로 플래그가 지정된 aBC에서 4개의 RAST 보고 가능 GN(RR-GN)을 식별했습니다. DIP의 미생물 식별 결과를 aMIAST의 표준 접종 준비 프로토콜(SIP)과 비교했습니다. 단형성 GN(+naBC)을 가진 204개의 +aBC 중에서, 4개의 RR-GN 중 하나가 SIP에 의해 105개의 +naBCs에서 확인되었다(E. coli: 50, K. pneumoniae: 20, P. aeruginosa: 9 및 A. baumannii complex: 26). 이 중 94%(98/105)는 DIP에 의해 정확하게 식별된 반면 주요 오류율과 매우 중요한 오류율은 각각 6%(7/105) 및 1.7%(4/240)였습니다. 일반적인 결과 연구 기간 동안 240 +naBCs는 DIP와 SIP를 모두 사용하여 aMIAST에 의해 식별되었습니다. 이 중 15%(36/240)+naBCs는 도금된 배지에서 하룻밤 동안 배양한 후 다미생물인 것으로 밝혀졌습니다. 204개의 +naBC 중 SIP에 의해 식별된 RR-GN의 비율은 51.5%(105/204)였다. 그 중 47.6%(50/105)는 대장균, 19%(20/105)는 K. 폐렴균, 8.6%(9/105)는 P. 녹농 균, 24.8%(26/105)는 A. baumannii 복합체였습니다.
