Whole Blood Assay with Dual Co-Stimulation for Antigen-Specific Analysis of Host Immunity to Fungal and Viral Pathogens

병원체 관련 항원을 사용한 전혈 자극 후 항원 특이적 면역 반응의 다중 모드 분석 대표 데이터 세트를 생성하기 위해 SARS-CoV-2 백신 접종을 받은 HSV-1 및 CMV 혈청 양성인 건강한 성인 기증자를 선택했습니다. 자극되지 않은 대조군 외에도 위에서 설명한 대로 다음과 같은 자극이 사용되었습니다: 단순 헤르페스 바이러스 1(HSV) 용해물(제조업체 권장, 미공개 데이터), 거대세포바이러스(CMV)pp65 22, 중증 급성 호흡기 증후군 코로나바이러스 2(SARS-CoV-2)Prot_S 5,23, 아스페르길루스 푸미가투스 용해물(유비쿼터스 환경 병원체)4,24, CRX-527(지질다당류를 기반으로 하는 톨 유사 수용체 4 자극제로, 그 자체로 T 세포를 활성화해서는 안 됨)25 및 CPI(CMV, 파라인플루엔자 바이러스 및 인플루엔자 바이러스 펩타이드로 구성된 CD4+ T 세포 활성화에 대한 양성 대조군)26. 유세포 분석 데이터와 gating 전략은 그림 3에 나와 있습니다. 일반적으로 희귀 세포 집단(50,000 - 100,000 lymphocytes)을 표적으로 삼을 때 가능한 한 많은 림프구를 측정하는 것이 권장되며, 측정의 정밀도와 신뢰성은 총 이벤트 수에 따라 달라진다27. 자극되지 않은 샘플은 개별 세포 subset(예: CD3+CD4+ 세포)의 gating에 사용됩니다. 메모리 T-세포 표현형을 위한 CD45RO 및 CCR7의 게이트는 전체 CD3+CD4+ 집단에서 설정한 다음 활성화 마커 양성 집단으로 전달되어야 하는데, 후자의 낮은 이벤트 수는 종종 뚜렷한 집단의 명확한 식별을 방해하기 때문입니다. 예를 들어 림프구 생존율의 차이를 설명하기 위해 개별 게이트에 대해 약간의 조정이 필요할 수 있습니다. CD154 (또는 CD40 리간드)의 상향 조절은 글로벌하고 일관되며 빠르게 유도 된 T-helper cell activation marker28,29로 설명되었습니다. IFN-γ은 가장 두드러진 1형 특이적 T 세포 활성화 마커 중 하나로 간주됩니다30,31. 중요한 것은 이 분석이 다양한 추가 활성화, 탈진 및 사이토카인 마커와 함께 테스트 및 발표되었다는 것입니다(표 4 및24 참조).
자극되지 않은 샘플에서 활성화 마커 양성 집단의 빈도는 비특이적 배경을 나타내며 항원 자극 빈도에서 차감되었습니다. 비특이적 배경을 뺀 후, 대표 공여자는 각각 0.75%(HSV), 0.09%(CMV), 0.06%(SARS-CoV-2), 0.34%(A. fumigatus), 0.00%(CRX-527) 및 1.21%(CPI) 특이 CD154+/CD3+CD4+ T-도우미 세포를 가지고 있었습니다. IFN-γ 발현도 유사하게 분석할 수 있으며, 그 결과 0.12%(HSV), 0.07%(CMV), 0.03%(SARS-CoV-2), 0.04%(A. fumigatus), 0%(CRX-527) 및 0.74%(CPI) IFN-γ+/CD3+CD4+ 세포가 생성됩니다.
T 세포 집단은 naive T 세포 (TN, CD45RO-CCR7+), 중앙 기억 T 세포 (TCM, CD45RO+CCR7+), 효과 기억 T 세포 (TEM, CD45RO+CCR7-) 및 효과 T 세포 (및 CD45RA, TE/TEMRA, CD45RO-CCR7-를 재발현하는 효과 기억 T 세포)로 더 세분화 할 수 있습니다. 전 세계 CD3+CD4+ T 세포 중 대표 공여체는 비자극 샘플을 사용하여 측정한 바와 같이 각각 38.34%의 TN, 38.33%의 TCM, 20.77%의 TEM 및 2.56%의 TE/TEMRA를 가지고 있었습니다(그림 3A). 그러나 항원 특이적 반응성 T-helper 세포(CD154+IFN-γ+) 중에서는 TCM과 TEM이 각각 22.14%와 73.97%로 가장 두드러진 부분집합이었다.
추가적인 백혈구 집단에 대한 데이터는 이 방법론을 사용하여 이전에 발표된 바 있다24. 사용된 항체 조합은 추가 참조를 위해 표 4 에 제시되어 있다.
또한 이 방법론의 잠재력을 최대한 발휘하기 위해 두 번째 자극 샘플 세트에 대해 IFN-γ ELISA를 수행했습니다(brefeldin A를 추가하지 않음). IFN-γ ELISA 키트의 검출 범위를 초과하는 것을 방지하기 위해 CPI 자극 샘플의 혈장을 1:4로 사전 희석했습니다. 다음 IFN-γ 농도를 측정하고 피험자의 혈액량의 mL당 정규화했습니다(즉, 자극 튜브와 ELISA 전 희석 모두에서 희석을 위해 보정됨: 0pg/mL(비자극), 69.4pg/mL(HSV), 471pg/mL(CMV), 17.8pg/mL(SARS-CoV-2), 61.9pg/mL(A. fumigatus), 34.0pg/mL(CRX-527) 및 1958pg/mL(CPI).
마지막으로, RNA는 일관된 결과로 동일한 샘플에서 분리되었습니다. 평균 수율은 719ng였으며 평균 흡광도는 260nm/280nm이며 1.98입니다.
전체적으로, 이 데이터 세트는 제시된 프로토콜이 최소 혈액량, 즉 다중 자극 및 판독 양식에 대해 총 8mL를 사용하여 다양한 감염 관련 항원의 다면적 판독 스펙트럼 및 수반 분석을 허용한다는 것을 보여줍니다.
항원 자극 전혈을 사용하여 백신 접종 반응을 추적하기 위한 전사 분석을 위한 대표 데이터 세트 항원 자극 전혈에 대해 수행된 전사 연구에 대한 원칙 증명으로, BNT162b2(SARS-CoV-2 mRNA 백신)32,33 초기 2회 접종 백신 시리즈 후 7-9개월 후 첫 번째 부스터 백신 접종 직전과 1개월 후 9명의 건강한 성인 피험자로부터 혈액을 수집했습니다. 500 μL의 무자극 및 Prot_S 자극 전혈의 평균 RNA 수율은 1.1μg의 고순도 RNA였으며 평균 260/280 흡수 비율은 1.99였습니다. nCounter 분석 후, RNA 수는 패널의 12개 하우스키핑 유전자(기하 평균)로 정규화되었습니다. 그 후, Prot_S-자극된 샘플과 자극되지 않은 배경 대조군에서 정규화된 mRNA 수의 비율을 각 피험자 및 유전자에 대해 측정했습니다. 백신 접종 후 측정과 백신 접종 전 측정의 중앙값 대 중앙값 비율을 결정하고, 재료 표에 나열된 소프트웨어 패키지를 사용하여 경로 농축 분석을 수행했습니다. 표준 경로의 풍부화는 절대 z-점수 값 ≥ 1.25 및 Benjamini-Hochberg 조정 p-값 &lt; 0.05에서 유의한 것으로 간주되었습니다. 현저하게 다르게 농축된 경로가 그림 4A에 요약되어 있으며, Prot_S에 대한 백신 접종 후 반응과 백신 접종 전 반응의 배경 조정 변화의 단순화된 네트워크가 그림 4B에 나와 있습니다. 또한, 부스터 백신 접종 후 항원 제시 세포 성숙 및 Prot_S 유도 T 세포 활성화와 관련된 대표 유전자의 더 강력한 배경 조정 유도가 그림 4C&lt; 나와 있습니다. 마지막으로, 대부분의 기증자에서 백신 접종 후 배경 조정된 Prot_S 특이적 1형 T-도우미 세포(CD69+IFN-γ+)의 증가는 두 번째 자극 튜브 세트를 사용한 유세포 분석으로 확인되었습니다(p = 0.03, 그림 4D).
전체 규모 및 소량 전혈 기반 면역분석 프로토콜에서 바이러스 반응성 T 세포 반응 비교 다음으로, 건강한 지원자에서 바이러스 반응성 CD154+IFN-γ+ T-도우미 세포(CD3+CD4+ 세포)의 빈도를 전체 크기(500 μL, WB) 및 소량(250 μL, WBS) 전혈 항원 자극 프로토콜을 사용하여 비교했습니다(그림 5). 이전에 보고된34개의 바와 같이, 이중 공동 자극(평균, WB 및 WBS에 대해 각각 0.010% 및 0.011%)에도 불구하고 CD154+IFN-γ+ 세포의 최소 비특이적 배경 주파수가 두 프로토콜 중 하나에서 관찰되었습니다. 주목할 점은, 항원 특이적 반응에서 비특이적 배경 반응이 제거되더라도, 이는 이전에 논의된 바와 같이 여전히 분석 부정확성 증가에 기여한다는 점이다27. 상승된 배경 신호(즉, &gt;0.07-0.1% CD154+ Th 세포 또는 &gt;0.05% CD154+IFN-γ+ 세포)는 시료 오염, 피험자의 급성 감염 또는 부적절한 사전 분석 시료 처리의 결과일 수 있습니다.
혈청 양성 공여체(n=5)의 평균 HSV 용해물 반응성 T 세포 빈도는 WB 및 WBS 시스템에서 각각 0.151% 및 0.107%인 반면, 혈청 음성 공여체(n=4)에서는 0.012% 및 0.004%였습니다. CMV pp65 펩타이드 풀에서 혈청 양성 공여체(n=4)는 0.041% 및 0.049%의 반응성 T 세포를 보인 반면, 혈청 음성 공여체(n=5)는 0.001% 및 0.004%였습니다. Lin의 일치 상관 계수는 HSV의 경우 0.868, CMV의 경우 0.985로 강한 상관 관계가 있음을 시사합니다(그림 5A). 특히, CMV 특이적 T 세포 검사는 최근 재활성화 사건이 없는 건강한 혈청 양성 피험자에서 음성일 수 있습니다. 총 및 항원 반응성 CD3+CD4+ T-helper 세포 레퍼토리는 모두 CCR7 및 CD45RO 발현을 기반으로 추가로 분화되었습니다(그림 5B). 안심할 수 있듯이, WB 및 WBS 프로토콜을 사용하여 얻은 결과는 전체 및 항원 반응 인구 모두에서 유사했습니다. 예상대로, 두 분석 모두에서 더 분화된 기억 Th 세포(즉, 효과기 기억 세포)의 비율은 전체 Th-세포 집단보다 항원 반응성 T 세포에서 더 높았습니다. 예상했던 대로, 단지 소수의 순진한 T 세포만이 바이러스 자극제에 의해 활성화되었다(그림 5B).
또한, 또 다른 실험 세트에서는 IFN-γ ELISA에 의해 자극된 배양 상층액을 분석했습니다(그림 5C). 최소한의 비특이적 배경이 관찰되었습니다(WB 및 WBS 프로토콜에서 각각 평균 1.29pg/mL 및 2.18pg/mL). HSV 양성 혈청 기증자의 혈액 샘플은 WB 및 WBS 시스템에서 각각 111pg/mL 및 125pg/mL의 평균 배경 조정 HSV 유도 IFN-γ 농도를 보여주었습니다. 대조적으로, 혈청 음성 샘플의 IFN-γ 농도는 두 시스템 모두에서 일관되게 10pg/mL 미만이었습니다(Lin의 일치 상관 계수 = 0.972, 그림 5C). 마찬가지로, CMV 혈청 양성 기증자의 혈액에 대한 pp65 자극은 WB 및 WBS 시스템에서 각각 258pg/mL 및 272pg/mL의 평균 IFN-γ 농도를 산출한 반면, 혈청 음성 샘플을 사용한 두 시스템 모두에서 최소 pp65 유도 IFN-γ 분비가 관찰되었습니다(Lin의 일치 상관 계수 = 0.953, 그림 5C).
