Abdominal Exam II: Percussion

1. Algemene percussie van de buik
<ol>
	<li>Leg de procedure uit aan de patiënt</li>
	<li>Voer lichte percussie uit over elk van de negen buikstreekjes.</li>
	<li>Tijdens het percusseren let op het gezicht van de patiënt voor tekenen van ongemak. Vraag de patiënt of ze enige gevoeligheid ervaren. Gevoeligheid tijdens percussie is abnormaal en kan duiden op ontsteking van de buikvlies.</li>
	<li>Luister naar de intensiteit, toonhoogte en duur van de percussietoon. Normaal gesproken zal een trommelend geluid, geproduceerd door lucht in de darmlussen, worden gehoord. Trommelend geluid is relatief lang, hoog en luid. Incidentele gebieden van dofheid (lager, korter en stiller geluid dan trommelen) worden geproduceerd door vloeistof en feces, en zijn ook normaal. Grote gebieden van dofheid suggereren een vergroot orgaan of een massa die verder moet worden beoordeeld door palpatie en aanvullende manoeuvres.</li>
	<li>Perceer de onderste voorste borstkas boven de costale randen. Normaal gesproken worden de milt en lever bedekt door het ribbenkastje, waarbij de leverrand incidenteel 1-2 cm onder de costale rand wordt gevonden. Dof geluid aan de rechterkant over de lever wordt verwacht. Aan de linkerkant zou trommelen over de gastrische luchtbel en de splenische flexuur van de dikke darm te horen moeten zijn.</li>
	<li>Perceer boven de symphysis pubica. Dofheid tijdens percussie in dit gebied duidt op een vergrote baarmoeder of verwijde blaas.</li>

2. Bepaal de leverspanwijdte
Een verhoogde leverspanwijdte tijdens percussie duidt op een vergrote lever, die kan onderliggen aan een verscheidenheid van pathologische processen. Het is essentieel om zowel de onderste als de bovenste randen van de leverdofheid te identificeren om onderscheid te maken tussen een vergrote lever (die een verhoogde spanwijdte heeft), en een lever die naar beneden is verplaatst als gevolg van chronische obstructieve longziekte (in welk geval de leverspanwijdte normaal is).

	<li>Als de leverspanwijdte is verhoogd, perceer lateraal en mediaal. Normale leverspanwijdte in de middensternallijn is 4-8 cm.</li>

3. Perceer de linker voorste borstkas om splenomegalie te detecteren
De milt bevindt zich iets achterachter de linker middenaxillaire lijn en produceert een ovale zone van dofheid tussen de 9e en 11e rib. Slechts een klein oppervlak van een normale milt is oppervlakkig genoeg om te worden gedetecteerd, en de miltdofheid wordt vaak verduisterd door gastrisch of colonisch trommelen. Echter, een vergrote milt is naar de middenlijn, anterieure en naar beneden uitgebreid, en kan worden gedetecteerd door twee speciale percussiemanoeuvres: Traube's ruimte percussie en/of Castell's manoeuvre.

4. Evaluatie van een protuberant buik.
Percussie is nuttig om de oorzaken van een protuberant buik te diagnosticeren. Trommelen over een protuberant buik duidt op luchtaccumulatie die kan worden veroorzaakt door een darmobstructie. Wanneer percussie over de flanken van een protuberant buik een dof geluid produceert, is dit consistent met vochtophoping of ascites.

Medische percussie is gebaseerd op het verschil in toonhoogte tussen de geluiden die worden veroorzaakt door op de lichaamswand te tikken. Het auditieve antwoord op tikken hangt af van de mate waarin de lichaamswand trilt, en wordt beïnvloed door onderliggende organen, de sterkte van de slag en de staat van de lichaamswand. Er zijn drie belangrijke medische percussie-geluiden: resonantie (gehoord over de longen), tympaniet (gehoord over de met lucht gevulde darmlussen) en stompheid (gehoord over vloeibare of vaste organen). Het contrast tussen stompheid versus tympaniet of resonantie maakt het mogelijk om de grootte en randen van organen en massa's te bepalen, evenals om vochtophoping en gebieden van consolidatie te identificeren. Percussie blijft een ingewikkeld onderdeel van de fysieke diagnose sinds het meer dan 200 jaar geleden werd geïntroduceerd, en is vooral nuttig bij het onderzoek van de longen en de buik.
Als onderdeel van een buikonderzoek volgt percussie op visuele inspectie en auscultatie. De onderzoeker moet eerst op elk van de negen buikgebieden percusseren (epigastrische regio, rechter hypochondrische regio, linker hypochondrische regio, navelregio, rechter lumbale regio, linker lumbale regio, hypogastrische regio, rechter inguinale regio en linker inguinale regio). Tenderheid veroorzaakt door percussie is abnormaal, en er moet worden gedacht aan peritoneale ontsteking. Aangezien de met lucht gevulde darmlussen zich het dichtst bij de buikwand bevinden, veroorzaakt percussie over de meeste delen van de buikholte een overwegend tympanisch geluid. De aanwezigheid van grote gebieden van stompheid moet aanleiding geven tot evaluatie voor organomegalie, intra-abdominale massa's of vocht.
Naast het beoordelen van de hoeveelheid en verdeling van buikgas, moet een buikonderzoek een schatting van de lever- en miltgrootte door percussie omvatten. Aangezien de lever en milt bedekt zijn door de ribbenkast, moet de onderzoeker ook over de onderste voorste borst percusseren. Normaal gesproken verwacht men een dof geluid te horen bij percussie van de rechter voorste borst over de lever, en een tympanisch geluid bij percussie van de linker voorste borst over de maag luchtbelletje en de splenische flexuur van de dikke darm (Figuur 1).

Percussie is een essentieel onderdeel van de abdominale onderzoek; daarom is het leren van de juiste techniek essentieel voor elke arts die snel, maar nauwkeurige diagnose van gastro-intestinale pathologieën wil stellen. Zoals u weet, is medische percussie gebaseerd op het verschil in toonhoogte tussen de geluiden die worden veroorzaakt door op de lichaamswand te tikken. Meet de afstand tussen de bovenste en onderste rand in centimeters. De leverspan is normaal 6-12 cm.Na de meting van de leverspan, percuteer om splenomegalie te detecteren. De twee manoeuvres hiervoor omvatten Percussie van de Traube's ruimte en de Castell's methode. Abdominal percussion is a physical exam technique that uses tapping to find differences in sound. The pitch changes because the body wall vibrates differently depending on the organs underneath, the strength of the tap, and the condition of the body wall. Doctors use this method to help judge organ size, find masses, and look for fluid or areas of tissue consolidation.

Three main percussion sounds are heard in medical exam. Resonance is heard over the lungs. Tympany is heard over air-filled bowel loops, and dullness is heard over fluid or solid organs. The contrast between dullness and the more hollow sounds helps the examiner define organ margins and detect abnormal areas.

During an abdominal exam, percussion comes after visual inspection and auscultation, which means listening with a stethoscope. The examiner first percusses the nine abdominal regions: epigastric, right hypochondriac, left hypochondriac, umbilical, right lumbar, left lumbar, hypogastric, right inguinal, and left inguinal. Tenderness with percussion is not normal and can suggest peritoneal inflammation.

Most of the abdomen normally gives a tympanic sound because air-filled bowel loops lie close to the abdominal wall. Large areas of dullness should raise concern for organ enlargement, intra-abdominal masses, or fluid. The exam should also estimate liver and spleen size by percussion, including over the lower anterior chest. Normally, dullness is heard over the right anterior chest where the liver lies, while the left anterior chest is usually tympanic over the gastric air bubble and the splenic flexure of the colon.
