The Mantel-Cox log-rank test is a nonparametric statistical method for comparing survival distribution curves between two groups.
It is typically employed in clinical research to evaluate treatment efficacy over time and guide further studies.
For instance, researchers can use this test to determine statistically significant differences between the survival curves of one group undergoing a novel therapy and another undergoing the control treatment.
The Mantel-Cox test calculates differences between observed and expected events across groups without assuming a specific survival time distribution. It is ideal for analyzing censored data in which not all subjects may experience the event of interest, like death or disease relapse.
Its limitation lies in its reliability on the proportional hazards assumption, which posits constant hazard ratios over time and may only sometimes be true. The results of this test could be misleading if the proportional hazard assumption is violated.
This is particularly true for studies with small sample sizes or high censoring rates.

In dergelijke gevallen kan het Cox-proportionele-hazardsmodel een beter alternatief zijn.

Ondanks zijn beperkingen biedt de Mantel-Cox log-rank-test een krachtige en eenvoudige manier om de impact van verschillende behandelingen op overleving te evalueren. De test houdt rekening met zowel de timing als de frequentie van gebeurtenissen, waardoor onderzoekers waardevolle inzichten kunnen verkrijgen in de effectiviteit van interventies. Dankzij de aanpasbaarheid en het vermogen om gecensureerde gegevens te verwerken, blijft de log-rank-test een essentieel hulpmiddel in overlevingsanalyse.

The test also does not rely on normally distributed survival times. That makes it suitable for many kinds of survival data. However, it assumes that the hazard ratios between groups stay proportional and constant over time, and that may not always be true.

If this assumption is violated, the results can be misleading. The test also needs enough events to give reliable findings. It is less effective when sample sizes are small or censoring is high, so a Cox proportional hazards model may be a better choice in those cases. Even with these limits, the Mantel-Cox log-rank test remains a simple and powerful tool for judging how treatments affect survival.
