This procedure outlines the steps for evaluating rectal temperature using an electronic thermometer.
Perform hand hygiene and wear clean gloves.
Instruct the patient to lie on their side with knees flexed, exposing only the anal region.
Then, clean the anal region if feces or secretions are present. Remove the soiled gloves, perform hand hygiene, and wear fresh gloves.
Next, take the designated rectal thermometer and attach a disposable probe cover to avoid contamination.
Lubricate the thermometer 1 to 1.5 inches to avoid trauma to the anus.
Instruct the patient to breathe and relax for optimal relaxation of the anal sphincter. Use the non-dominant hand to separate the patient's buttocks and expose the anus.
Insert the thermometer gently about 1-1.5 inch or 3-4 centimeters towards the umbilicus. Wait for the beep, remove probe, and take note of the reading.
Clean the anal area to remove lubrication or feces and ensure patient comfort.
Discard the probe cover and clean the stem with an alcohol swab.
Remove the gloves, perform hand hygiene, and document the procedure.
