Focus charting uses three columns to organize patient information. The first column records the date and time of each entry. The second column, called the focus column, documents specific patient concerns such as nursing diagnoses, signs or symptoms, patient behaviors, special needs, or acute changes in condition. The third column contains progress notes using the DAR format.

DAR stands for data, action, and response. Data includes subjective and objective information describing the focus, such as vital signs and behavioral observations. Action documents nursing interventions tailored to the patient's condition and updates the care plan based on evaluations. Response describes how the patient reacted to the nursing or medical care provided.

Focus charting minimizes duplicate entries, improves data organization, and encourages consistent documentation practices across the care team. It promotes clear communication among healthcare providers by organizing information systematically. The structured format helps nurses document patient responses regularly and adapt care plans based on nursing assessments and evaluations.

The primary disadvantage is that nurses may struggle to organize information in the DAR format due to time constraints in busy clinical settings. This format requires careful consideration and documentation of data, actions, and responses, which can be challenging to accomplish efficiently when managing multiple patients simultaneously.

The action category represents the implementation phase of the nursing process, encompassing planning and execution of nursing interventions. It documents actions taken in response to the patient focus, customized to the patient's condition and rooted in nursing assessments. The care plan may be adjusted based on these assessments to ensure optimal patient care.

The data category documents subjective and objective information related to the patient focus. This includes vital signs, patient behavioral observations, laboratory results, and other relevant clinical information. This assessment-like section provides the foundation for determining appropriate nursing actions and evaluating patient responses to care.
