A thorough airway assessment is critical before decannulation, including a trial period with the tube corked to ensure safety.

The procedure begins with thorough hand-washing and donning gloves.

First, explain the procedure to the patient and monitor their vital signs.

Next, suction the tracheostomy to clear tracheal secretions, then loosen the tracheostomy tapes and remove any visible sutures securing it.

Deflate the tracheostomy cuff and gently pull the tracheostomy tube outward.

After removing the tube, apply a sterile occlusive dressing and monitor the patient for bleeding.

Change the dressing if it becomes soiled or wet.

Monitor the patient's respiratory status and oxygen saturation for signs of breathing difficulties, and administer an alternative oxygen delivery system if necessary.
