Renal clearance, pivotal in drug elimination, can be affected by drug distribution and interactions.
Drugs with a large distribution volume or bound to plasma proteins are poorly excreted in the urine. Like macromolecules, they are not filtered through the glomerulus, influencing their half-lives.
On the other hand, unbound drugs are excreted better.
For instance, oxytetracycline, which is less protein-bound, has better clearance than highly protein-bound doxycycline.
Displacement of a drug bound to plasma proteins can increase its renal clearance. Phenylbutazone displaces warfarin from binding sites, increasing the free warfarin concentration and accelerating renal clearance.
Urine acidification expedites the excretion of basic drugs. In contrast, alkalinization of urine favors the excretion of acidic drugs.
The use of diuretics increases the elimination of drugs affected by urine flow rate.
