In patients with renal impairment, drugs exhibit altered pharmacokinetics.
Reduced renal clearance and elimination rate result in prolonged elimination half-life, necessitating dosage adjustments.
The dosage regimen need not be changed when fu, the fraction of drug excreted unchanged is ≤ 0.3, and the renal function, RF, is ≥ 0.7 of normal.
This assumption relies on metabolite inactivity, unchanged binding characteristics, and sustained drug availability during renal failure.
However, as fu approaches unity and RF nears zero, elimination significantly slows, requiring substantial dose reduction.
Nonrenal clearance gains prominence under these conditions.
A simple equation calculates the required dose in patients with renal impairment.
Similarly, the dosing interval can be computed.
