Hepatic clearance is the volume of blood cleared of a drug by the liver per unit of time. It equals total body clearance minus renal clearance.
Most drugs undergo restrictive clearance, proportional to the unbound drug fraction. The highly protein-bound drugs, when displaced, exhibit a substantial increase in clearance to low-protein-binding drugs.
Conversely, nonrestrictive clearance applies to drugs extracted by the liver irrespective of protein binding. During metabolism, these are more easily stripped from the protein.
Hepatic blood flow, intrinsic clearance, and protein binding share a proportional relationship for restrictively cleared drugs.
If intrinsic clearance is smaller than blood flow, hepatic clearance is proportional to intrinsic clearance of the free and bound drugs.
If intrinsic clearance for a drug surpasses blood flow, the equation reduces, and hepatic clearance becomes dependent on the hepatic blood flow, irrespective of protein binding
