The Michaelis-Menten equation describes capacity-limited kinetics in drug metabolism.
It determines the rate of decline of drug concentration over time, with Vmax and KM as key parameters.
Vmax is the maximum process rate, while KM, the Michaelis constant, shows the drug concentration at half this maximum rate.
Three scenarios arise based on the relationship between KM and Cp, the plasma drug concentration.
When KM equals Cp, the process rate is half its maximum.
When KM is greater than Cp, the process rate mimics first-order elimination typically in most drugs at therapeutic levels.
When KM is less than Cp, the process maintains a constant rate, similar to zero-order elimination. Certain drugs like salicylates and phenytoin, at elevated therapeutic doses, saturate hepatic mixed-function oxidases, following zero-order kinetics.
The Michaelis-Menten plot depicts an initial linear rate increase with concentration, transitioning to mixed-order at higher concentrations, and finally reaching a plateau at Vmax.
