Some conditions cause complex changes in muscle tone. These include spasticity, paratonia, and dystonia.
Spasticity is a velocity-dependent increase in muscle tone that depends on the rate of muscle stretch. The quicker the movement, the greater the resistance.
It results from upper motor neuron damage, seen in stroke, cerebral palsy, and multiple sclerosis.
Unlike spasticity, paratonia, also called gegenhalten, is not velocity dependent and shows variable resistance to passive movement that increases with the examiner’s applied force.
It is linked to frontal lobe dysfunction and is often seen in dementia.
Dystonia develops from abnormal signaling in motor control networks involving the motor cortex, basal ganglia, and cerebellum, leading to impaired inhibition and excessive muscle activation that causes sustained contractions, twisting movements, and abnormal postures.
